VANKOMİSİN DİRENÇLİ ENTEROKOKLARDA DİRENCİN MOLEKÜLER YÖNTEMLERLE ARAŞTIRILMASI

An Investigation of Resistance Using Molecular Methods in Vancomycin Resistant Enterococcus

Özet

Doğada yaygın olarak bulunan, insan ve hayvanların gastrointestinal f lorasının üyesi olan, sıklıkla kullanılan antibi- yotiklere direnç geliştirebilen enterokoklar, hastanede yatan riskli hasta grupları için önemli patojenlerdir. Bu çalışmada has- tanemizdeki yoğun bakım hastalarında vankomisin dirençli enterokoklarda (VRE) direnç tiplerinin moleküler yöntemle araş- tırılması amaçlanmıştır. Çalışmaya 417 rektal sürüntü örneği dahil edilmiştir. Konvansiyonel bakteriyolojik çalışmaların ardından tanımlama, antibiyotik duyarlılık ve minimal inhibitör konsantrasyon (MİK) verileri otomatize sistemle (VITEK 2) elde edilmiştir. VRE suşlarında vanA ve vanB gen bölgesi varlığının araştırılmasında; ‘Mag 16 bacterial DNA ekstraksiyon kiti’ (Fluorion, Iontek) ve Fluorion VRE QLP 1.0 (Iontek) gerçek zamanlı polimeraz zincir reaksiyon (q-PZR) kitleri üretici f irma önerileri doğrultu- sunda çalışılmıştır. Yoğun bakım hastalarından alınarak laboratuvara gönderilen rektal sürüntü örneklerinden % 11.5 (n=48) oranında VRE kolonizasyonu tespit edilmiştir. VRE suşlarının % 98’inin (n=47) E.f aecium, % 2’sinin (n=1) E.f aecalis olduğu belir- lenmiştir. İzolatların duyarlılık oranları tetrasiklin, klindamisin, amikasin, siprof loksasin, moksif loksasin, imipenem, strepto- misin, gentamisin ve tigesiklin için sırasıyla % 6.3, 16.7, 18.8, 20.8. 29.2, 29.2, 33.3, 45.8 ve 100 olarak saptanmıştır. Çalışma kapsamında incelenen 48 VRE suşunun qPZR ile moleküler direnç geni araştırmaları sonucunda tüm suşların van A tipi dirence sahip olduğu saptanmıştır. Tüm dünyada olduğu gibi ülkemizde de VRE yayılımı ve direnç tiplerinin izlenmesi hastane inf eksiyonlarının önlen- mesi açısından önemlidir. Hastanemizde yapılan çalışmalarda diğer çalışmalarla uyumlu olarak, vanA tipi dirence sahip E.f eacium izole edilmiştir. Ancak suşların klonal ilişkisi henüz ortaya konamamıştır. Bu ilişkinin gösterilmesi hastane inf ek- siyonlarının izlenmesi ve önlenmesine katkı sağlayacaktır.

Abstract

Enterococci; extensively present in nature, and member of human and animal gastrointestinal f lora are an important pathogen f or the hospitalized risky patient groups and are capable of developing resistance to f requently used antibiotics. In this study, vancomycin resistance genotypes of enterococci (VRE) isolated f rom intensive care unit patients in our hospital was investigated by using molecular method. A total of 417 rectal swab specimens were included to the present study. Af ter conventional bacteriological studies; identif ication of bacteria at species level, as well as determination of antibiotic susceptibilities and minimal inhibitor concent- rations (MIC) were done using an automated system (VITEK 2). Presence of vanA and vanB genes in VRE strains were studied using Mag 16 bacterial DNA extraction kit (Fluorion, Iontek) and real time polymerase chain reaction (qPCR) kit (Fluorion VRE QLP 1.0 – Iontek) according to manuf acturer’s instructions. VRE colonization was determined among 11.5 % (n=48) of the rectal swab specimens collected f rom the intensive care unit patients and sent to our laboratory. It was f ound that 98 % (n=47) of VRE isolates were E.f aecium and 2 % (n=1) was E.f aecalis. The susceptibility rates of these isolates f or tetracyclin, clindamycin, erithromycin, amikacin, ciprof loxacin, moxi- f loxacin, imipenem streptomycin, gentamycin and tigecyline were 6.3, 16.7, 18.8, 20.8. 29.2, 29.2, 33.3, 45.8 and 100 % respec- tively. Presence of vancomycin resistance gene tested by qPCR showed that all 48 VRE strains had vanA type resistance. The spread of VRE and related resistance types’ monitorization in our country and the entire world is important to prevent nosocomial inf ections. During the studies in our hospital, compatible to other studies, E.f eacium having vanA type resistance was isolated. However, the strains’ clonal relation could not be evaluated yet. The demonstration of this relationship will contribute to prevent and to observe hospital inf ections.
