İshalli hastaların dışkı örneklerinde Clostridium difficile toksin A/B sıklığının araştırılması

Özet

Clostridium difficile, antibiyotik ile ilişkili ishallerin en önemli etkenlerindendir. Toksijenik C.difficile suşları, asempto- matik kolonizasyondan, pseudomembranöz kolite kadar değişen farklı klinik semptomlara neden olmaktadır. C.difficile ile ilişkili kolit tanısında dışkı örneklerinde toksin üretimini belirleyen testler yaygın olarak kullanılmaktadır. Çalışmamızda ishalli hastalara ait dışkı örneklerinde C.difficile toksin A/B varlığının belirlenmesi amaçlanmıştır. Toksin A/B varlığının belirlenmesi, enzim immünoassay yöntemi (ELISA) ile yapılmıştır. Bu amaçla, Serazym Clostridium difficile Toxin A/B(Seramun, Almanya) ticari kiti kullanılmıştır. İncelenen 592 adet dışkı örneğinde C.difficile toksin A/B pozitifliği % 24 olarak tespit edilmiştir. Kan ve dışkı lökosit sayılarının Toksin A/B pozitif hastalarda toksin negatif hastalara göre anlamlı düzeyde düşük olduğu belirlenmiştir (p< 0.01, p=0.012). C.difficile toksin A/B pozitiflik oranı en sık hematoloji-onkoloji, kemik iliği nakil ünitelerindeki hastalara ait dışkı örneklerinde tespit edilmiştir. C.difficile infeksiyonunun ishalli hastalardaki dışkı örneklerinde yüksek oranda bulunduğu tespit edilmiştir. Özellikle antibiyotiklerin yoğun olarak kullanıldığı ve immünsupresif hastaların bulunduğu ünitelerde C.difficile infeksiyonlarının daha sık görüldüğü tespit edilmiştir. Bu nedenle, C.difficile’nin, ishal ayırıcı tanısında mutlaka düşünülmesi gereken bir patojen olduğu ve tanısına yönelik testlerin yapılmasının uygun olacağı düşünülmüştür. C.difficile ile ilişkili ishallerde antibiyotik kullanımının yüksek oranda olması nedeniyle, hastanelerin belirli zaman aralıklarında antibiyotik kullanım stratejilerini gözden geçirmeleri gerekmektedir.

Abstract

Clostridium difficile is one of the most important causes of diarrhea associated with antibiotics. Toxigenic strains of C.difficile lead to various clinical symptoms, ranging from asymptomatic colonization to pseudomembranous colitis. The most widely used test for diagnosing C.difficile associated colitis is a test that detects toxins produced by C.difficile in stool samples. In our study, stool samples of 592 patients with diarrhea were studied for prevalence of C.difficile Toxin A/B. To investigate the presence of Toxin A/B, the enzyme immunoassay method (ELISA) was carried out using ile Serazym Clostridium difficile Toxin A/B kit (Seramun, Germany). In the stool samples examined, 24 % C.difficile toxin A/B positivity was found in total 592 stool examples. The number of leukocytes in the blood and stool samples was significantly lower in Toxin A/B positive patients than those of Toxin A/B negative patients (p< 0.01, p=0.012). The rate of C.difficile Toxin A/B positivity was more frequently detected in patients stool samples the divisions of hematology-oncology and bone marrow transplantation. C.difficile infection rate in patients stool samples showing symptoms of diarrhea was high. In divisions which antibio- tics were widely used and patients were immunsuppressed, C.difficile infections were more common. C.difficile should be considered as a pathogen in the differential diagnosis of diarrhea and diagnostic testing might be appropriate. In addition, since the use of antibiotics is high in C.difficile associated diarrhea, hospitals should revise their antibiotic strategies periodically.
