Thrombotic Thrombocytopenic Purpura Associated with Cyclocyporin Use in a Liver Transplant Patient

Özet

Karaciğerkullanılan immünsüpresif ilaçlara bağlı olarak pek çokkomplikasyonkomplikasyonlardan biri olan trombotik mikroanjiyopati(TMA) oldukça nadir görülen ve tedavi edilmediğitakdirde hayatı tehdit eden bir durumdur. Literatürüincelediğimizde karaciğer nakli sonrası kalsinörininhibitörü kullanımına bağlı gelişen az sayıda trombotiktrombositopenik purpura (TTP) olgusuna rastladık. Yinebizim bilgilerimize göre bu olgu ülkemizde karaciğernakli sonrası siklosporin kullanımına bağlı gelişen ilkTTP olgusudur. Karaciğer nakli olan hastamızdasiklosporin kullanmakta iken TTP tablosu gelişmiş oluppulse steroid ve plazma exchange tedavisi ile hastalıkkontrol altına alınmıştır

Abstract

Many immunosuppressive drugs given to patients with livertransplantation. Thrombotic microangiopathy (TMA),which is one of those complications, is encounteredvery rarely and if untreated, it endangers life. Whenwe reviewed the literature, we came across to a fewthrombotic thrombocytopenic purpura (TTP) cases,which have developed due to calcineurin inhibitor useafter the liver transplantation. And according to ourknowledge, this present case was the first TTP case,which occurred due to cyclosporine use after the livertransplantation. While our patient with livertransplantation was receiving cyclosporine, clinicalpicture of TTP was developed, and the disease wascontrolled by using pulse steroid and performingplasma exchange treatment
