Özet
Tüberkülozda antitüberküloz ilaçlara direnç sürveyansı, bu hastalığın kontrol programının önemli bir parçasıdır. Bu çalışmada, laboratuvarımızda 2009-2012 yılları arasında izole edilmiş Mycobacterium tuberculosis kompleksi suşlarının pri- mer antitüberküloz ilaçlara (izoniazid, rifampisin, etambutol ve streptomisin) duyarlıklarının değerlendirilmesi ve direnç paternlerinin belirlenmesi amaçlanmıştır. Mikobakteri kültürü ve duyarlılık testleri BACTEC MGIT 960 (Becton Dickinson, ABD) sistemi ile üretici firmanın önerileri doğrultusunda çalışılmıştır. 2009 yılında 88, 2010 yılında 49, 2011 yılında 84 ve 2012 yılında 54 suş olmak üzere toplam 275 suş değerlendirmeye alınmıştır. Suşların 205’i (% 74.5) ilaçların tümüne duyar- lı, 70’i (% 25.5) en az bir ilaca dirençli ve üçü (% 1.1) ilaçların tümüne dirençli bulunmuştur. Değerlendirmeye alınan suşlar için genel direnç oranları; izoniazid % 14.9, rifampisin % 6.2, etambutol % 7.9, streptomisin % 9.5 ve çok ilaca direnç (ÇİD) % 4 olarak belirlenmiştir. Antitüberküloz ilaçlara direncin belirlenmesi, direnç gelişimi ve yayılımının önlenmesi ve uygun ve zamanında tedaviye başlanması, tedavi giderlerinin azaltılması ve ÇİD tüberküloz yayılımının önlenmesinde son derece önem- lidir.
Abstract
Surveillance of drug resistance in tuberculosis is an important component of the tuberculosis control program. The aim of this study was to determine the susceptibilities of Mycobacterium tuberculosis complex isolates to primary antituberculous (anti- TB) drugs and their resistance patterns. BACTEC MGIT 960 (Becton Dickinson, USA) system was used for mycobacterial culture and susceptibility testing according to the manufacturer’s recommendations. Of the 275 isolates evaluated: 88 isolated in 2009, 49 in 2010, 84 in 2011 and 54 in 2012. A total of 205 (74.5 %) were susceptible to all the drugs tested, 70 (25.5 %) were resistant to at least one drug and three were 1.1 % resistant to all tested drugs. The total resistance rates for isoniazid, rifampicin, ethambutol and streptomycin were 14.9 %, 6.2 %, 7.9 % and 9.5 %, respectively. Multiple-drug resistance (MDR) was 4 %. The prompt detection of anti-TB drug resistance is essential for controlling the development and spread of MDR-TB because detecti- on facilitates the appropriate and timely delivery of anti-TB therapy, reduces the overall cost of treatment, minimizes the risk of developing further resistance and limits the spread of MDR-TB.
