EFFECT OF TWO DIFFERENT APPLIANCES IN THE MANAGEMENT OF PATIENTS DIAGNOSED WITH SLEEP BRUXISM BY POLYSOMNOGRAPHY

Özet
Amaç: Bu çalışmada, uyku bruksizminin tedavisinde bir stabilizasyon splintinin ve güncel bir anterior kapanışı yeniden programlama splintinin (NTI-tss; anterior bite reprogrammer splint) etkileri, polisomnografi kullanılarak incelenmiştir. Stabilizasyon splinti tüm maksiller arkın okluzal yüzeylerini kaplarken; güncel splint, karşıt kesici dişler arasında sadece bir temas noktası oluşturacak şekilde maxiller ve mandibular kesici dişleri kaplamıştır. Bireyler ve Yöntem: Polisomnografi elektromiyografi ile uyku bruksizmi olduğu teyit edilen yirmi bruksizm hastası çalışmaya alınmış ve rastgele stabilizasyon grubuna (9 hasta) veya anterior bite reprogrammer splint grubuna (11 hasta) ayrılmıştır. Katılımcılara splintleri 4 ay kullanmaları öğütlenmiştir. Uyku esnasındaki kas aktiviteleri ölçülmüştür. İlk 2 gece başlangıç ölçümleri alınmış, 3. gece ölçümleri ise düzenli splint kullanımından 4 ay sonra gerçekleştirilmiştir. Bruksizm dönemleri, süreleri ve bruksizm şiddeti analiz edilmiştir. Bulgular: Uyku bruksizminin frekansında ve süresinde her iki splintle de istatistiksel olarak anlamlı bir azalma gözlenmemiştir. Bununla birlikte, anterior bite reprogrammer splint masseter ve temporal kasların aktivitesini azaltırken, stabilizasyon splinti bu parametrede herhangi bir değişiklik yaratmamıştır. Sonuç: Bu çalışmanın bulguları, bruksizm tedavisinde güncel anterior bite reprogrammer splintin geçerli bir alternative olduğunu göstermiştir.
Abstract
Background and Aim: This study investigating the effect of a stabilization splint and a novel anterior bite reprogrammer splint (NTI-tss; an anterior bite reprogrammer splint) on sleep bruxism by using polysomnography. The stabilization splint covers the occlusal surface of the complete maxillary dental arch, whereas the novel splint covers the maxillary or mandibular incisors, creating only a contact point with the opposing incisors. Subjects and Methods: Twenty sleep bruxism patients that were confirmed to have sleep bruxism by polysomnography electromyography were recruited and randomly divided into a stabilization splint group (9 patients) and an anterior bite reprogrammer splint group (11 patients). The participants were instructed to wear the splint during sleep for 4 months. Masticatory muscle activity was measured during sleep. The first 2 nights were base nights, and the third study night was conducted 4 months after regular use of the splints. The number of bruxism episodes and the duration and intensity of sleep bruxism were analyzed. Results: No statistically significant reduction in the frequency and duration of sleep bruxism was observed with either type of splint. However, the anterior bite reprogrammer splint reduced the intensity of the masseter and temporal muscle activity, whereas the stabilization splint showed no effect on this parameter. Conclusion: The findings of this study demonstrate that the anterior bite reprogrammer splint is a viable alternative for the management of bruxism.
