Özet

Orbital duvar kırıkları, künt travma ile oluşan intraokülerbasıncın ani yükselmesi nedeni ile meydana gelir.Çocuklarda en yaygın olarak trapdoor tipi blow-outkırığı görülür. Blow out kırıkları en sık inferior(maxiller) ve medial (etmoid) duvarlarda meydana gelir.İnferior rektus kası sıkıştığı için inferior duvarkırıklarında yukarı bakış kısıtlılığı, enoftalmus, pitozis,aynı tarafta yanakta ağrı ve diplopi görülür. 12 yaşındakız hasta sol göz ve yüz travması sonrası acilegetiriliyor, hastada diplopi ve yukarı bakış kısıtlılığıolması üzerine çekilen maksillofasiyal bilgisayarlıtomografi (BT)'de orbita tabanında trapdoor şeklindefraktür saptanıyor. Hasta medikal tedavi sonrası, erkendönemde travma sonrası üçüncü gün cerrahiyeyönlendirilerek opere edildi. Bu tip kırıkların gözdenkaçırılmaması ve erken dönemde yapılan cerrahioperasyonun önemini vurgulamak amacı ile bu vakasunulmuştur

Abstract

After a blunt trauma, by a sudden increase inintraoccular pressure orbital wall fracture results withfractures in the thin walls. In children, the mostcommon type of blow-out fracture is trapdoor. Blowout fracture occurs most often in the inferior(maxillary) and medial (ethmoid) wall. In the inferiorwall fracture limitation glance up, enophtalmus,ptosis, pain at the cheek on the same side and diplopiaoccur because of inferior rectus muscle tightness. A12-year-old girl was brought to emergency departmentwith the left eye and facial injuries after facial trauma,and then for diplopia and limitation of upgazemaxillofacial computed tomography (CT) wasperformed. The CT findings showed trapdoor orbitalfloor fracture. After medical treatment, on the thirdday after the trauma, the patient was operated. Thiscase is presented to emphasize the importance ofsurgery performed during the early stages of traumaand not to overlook this type of fracture
