Hastane infeksiyonu etkeni Acinetobacter baumannii suşlarının antimikrobiyal direnç oranlarının araştırılması

Özet

Bu çalışmada, hastanemizde yatan hastaların örneklerinden izole edilen Acinetobacter baumannii izolatlarının çeşitli antibiyotiklere in vitro etkinliğinin belirlenmesi amaçlanmıştır. Mart 2011-Kasım 2012 tarihleri arasında Kayseri Eğitim ve Araştırma Hastanesi kliniklerinde yatan, Centers for Disease Control and Prevention (CDC) kriterlerine göre nozokomiyal infeksiyon tanısı konulan hastaların trakeal aspirat, kan, idrar, balgam ve yara yeri örneklerinden izole edilen 161 A.baumannii suşu çalışmaya alınmıştır. Birden fazla üremesi olan hastaların sadece ilk üremesi çalışmaya dahil edilmiştir. İzole edilen mikroorganizmaların tanımlanması ve antibiyotik duyarlılıkları VITEK 2 ID-AST kart (bioMerieux, France) otomatize sistem kullanılarak saptanmıştır. Suşlarda kolistine direnç saptanmamış, tigesikline % 11, gentamisine % 54, amikasine % 59, meropenem ve imipeneme % 91, sefoperazon-sulbaktam ve siprofloksasine % 92, levofloksasine % 94, sefepime % 95 ve piperasilin-tazobaktama % 97 oranında direnç saptanmıştır. Sonuç olarak, hastanemizde A.baumannii suşlarının kolistin hariç birçok antibiyotiğe yüksek direnç gösterdiği saptanmıştır. Dirençli suşların hastane ortamında yayılmasının ve antibiyotik direncinin diğer bakterilere aktarımının önlenmesi için infeksiyon kontrol önlemlerine katı bir şekilde uyumun sağlanması ve akılcı antibiyotik kullanım politikalarının uygulanması gerekmektedir.

Abstract

The aim of this study is to determine in vitro antibiotic resistance of Acinetobacter baumannii strains isolated from various specimens of hospitalized patients in our hospital. A retrospective study from March 2011 to November 2012 was conducted in Kayseri Training and Research Hospital. The diagnosis of nosocomial A.baumannii infection was determined according to Centers for Disease Control and Prevention (CDC) criteria. Antibiotic resistance paterns of 161 A.baumannii strains isolated from tracheal aspirate, blood, urine, sputum, and wound specimens of patients was performed by automated VITEK 2 ID-AST card (bioMerieux, France) system. Only the first specimen tested for each patient was included. Colistin resistance was not detected. The antibiotic resistance rates of the strains were found to be 11 % for tigecycline, 54 % for gentamicin, 59 % for amikacin, 91 % for imipenem and meropenem, 92 % for cefoperazone-sulbactam and ciprofloxacin, 94 % for levofloxacin, 95 % for cefepime and 97 % for piperacillin- tazobactam. In conclusion, the A.baumannii strains were found to be resistant to many antibiotics, except colistin and tigecycline in our hospital. Applying strict infection control programs for these strains and rational antibiotic usage policies are of utmost importance in order to prevent further spread of resistant strains and transfer of antibiotic resistance to other bacteria in the hospital setting.
