Özet

Malign plevral mezotelyoma, sıklıkla asbest maruziyetisonucu oluşan, plevradan köken alan lokal, invazifkarakterli agresif bir tümördür. Genellikle nefes darlığıve göğüs ağrısı ile prezente olan malign plevralmezotelyoma, nadiren spontan pnömotoraks ile deprezente olabilmektedir. Biz, pnömotoraks ile prezenteolan malign plevral mezotelyoma olgumuzu sunmayıamaçladık. Yaklaşık 1 yıl önce 2 ayda 3 kez spontanpnömotoraks nedeni ile sol tüp torakostomi uygulananhasta, operasyonu kabul etmemesi nedeni ile opereedilemedi. Nefes darlığı nedeniyle çekilen torakstomografisindekalınlaşma ve apikal büller saptanması üzerine 4. kezpnömotoraks nedeniyle hasta yatırıldı. Uzamış havakaçağı nedeni ile cerrahi kararı verildi. Alt lobta bir adetyaklaşık 1 cm'lik nodül saptandı ve wedge rezeksiyon ileeksize edildi. Patoloji sonucu malign plevralmezotelyoma olarak raporlandı. Yaşlı hastalardasaptanan pnömotoraks durumlarında özellikle malignplevral mezotelyomanın endemik olduğu bölgelerdemalign plevral mezotelyomanın akılda tutulmasıgerektiğini düşünmekteyiz

Abstract

Malignant pleural mesothelioma is a pleuraloriginated tumor caused by prolonged asbestosparticles inhalation exposure. It is uncommon yet alethal cause of cancer. It generally causes slow-onsetdyspnea and chest pain and rarely presents asspontaneous pneumothorax. We present a malignantmesothelioma case who was presented withspontaneous pneumothorax. A 73 year old malepatient who had recurrent pneumothorax (3 times in 2months), was admitted to the emergency service dueto dyspnea 1 year ago. Chest tube was applied afterhydropneumothorax and pleural thickening on the lefthemithorax were detected by thorax computedtomography (CT). Surgical operation was planned dueto prolonged air leakage and the lack of lungexpansion after two weeks follow up of the chest tube.During the operation a pulmonary nodule with 1 cm indiameter in the left lower lobe was examined andwedge resection was applied. The pathologicalexamination was reported as malignant pleurealmesothelioma, in pulmonary nodule and parietalpleura samples. Patient was consulted to oncologyclinic. We think that malignant pleureal mesotheliomashould be kept in mind in old patients who admit tohospital with recurrent spontaneous pneumothoraxespecially in endemic areas
