Elektrokonvulsif Terapinin Hematolojik Parametreler Üzerindeki Etkisi

Özet

Amaç: Elektrokonvulsif terapi (EKT), beyin dokusunun kont- rollü olarak elektrik akımı ile uyarılması sonucu jeneralize kon- vulsiyonlar oluşturulması esasına dayalı bir psikiyatrik tedavi yöntemidir. En yaygın kullanım alanı ilaç tedavisine cevap ver- meyen depresyon olguları olmakla beraber mani, katatoni, affek- tif bozukluklarla seyreden şizofreni, parkinson hastalığı ve nöro- leptik malign sendrom (NMS) gibi birçok hastalığın tedavisinde de etkilidir. EKT öncesi tam kan sayımı rutin olarak bakılsa da, EKT için herhangi bir hematolojik kontrendikasyon bulunma- maktadır. Bu yaptığımız çalışmanın amacı EKT tedavisinin he- mogram parametreleri üzerindeki etkisini incelemektir. Yöntemler: Bu çalışmaya Harran Üniversitesi Tıp Fakültesi Psi- kiyatri Anabilim Dalı Kliniği’nde yatan ve EKT uygulanan 30 hasta alınmıştır. Hastaların tedavi öncesi ve sonrası hemogram parametreleri kaydedilmiştir. Bulgular: Çalışmaya dahil edilen 25 hastanın, 19 tanesi (%76) kadın, 6 tanesi (%24) erkektir. Hastaların yaşları 16 ile 56 arasın- da değişmekte olup; yaş ortalaması 33,12±12,06 olarak saptan- mıştır. Ortalama EKT sayısı 9,04±3,12 olarak değerlendirildi. EKT öncesi ve sonrası karşılaştırılan hemogram parametrelerinin istatistiksel analiz sonuçlarına göre, RBC (eritrosit hücrelerinin sayısı) ve MCH (eritrosit hücrelerindeki ortalama hemoglobin miktarı) değerlerinde anlamlı değişiklik saptanmıştır. RBC orta- lama değerinde 4,90’dan 4,68’e düşüş gözükmektedir (p=0,018). MCH ortalama değerinde ise 27,37’den 27,85’ yükseliş tespit edilmiştir (p=0,036). Diğer hemogram parametrelerindeki deği- şiklikler ise istatistiksel açıdan önem arz etmemektedir. Sonuç: EKT, yan etkisi oldukça az, etkili, güvenli ve kolayca uygulanabilir bir tedavi yöntemidir. Bu çalışma EKT’nin birçok hemogram parametresi üzerinde anlamlı bir istatistiksel değişik- lik yapmadığını göstermektedir. Çalışmamızda anlamlı değişiklik gösteren 2 parametre (RBC ve MCH) bulunmuştur. EKT’nin kırmızı kan hücrelerinde ve hemoglobin miktarlarında nasıl bir değişiklik yaptığı ile ilgili net verilere ulaşmak için, daha fazla sayıda çalışma yapılmasına ihtiyaç vardır.

Abstract

Objective: Electroconvulsive therapy (ECT), a method of psy- chiatric treatment based on the establishment of generalized con- vulsions, results in a controlled stimulation of the brain tissue by an electrical current. The aim of this study was to examine the impact of the ECT treatment on hemogram parameters. Al- though the most common areas of use are depressive episodes that do not respond to medication, it is also effective in the treat- ment of many diseases such as mania, catatonia, schizophrenia with affective disorders, parkinson's disease and neuroleptic ma- lignant syndrome (NMS). Though full blood count before ECT routine, there is no hematologic contraindication for ECT. Methods: This study included 30 patients who were admitted to the Department of Psychiatry of Harran University Medical Faculty and who underwent ECT. Hemogram parameters were recorded before and after treatment of patients. Results: Of the 25 patients included in the study, 19 (76%) were female, 6 (24%) were male. The ages of the patients ranged from 16 to 56, and the mean age was 33.12±12.06. The mean number of the ECT seances was 9.04±3.12. Num- ber of red blood cells (RBC) and mean hemoglobin amount in erythrocyte cells (MCH) were found to be significantly chang- es, according to the results of statistical analysis of hemogram parameters before and after ECT. The RBC average value ap- peared to decline from 4.90 to 4.68 (p=0.018). But the average value of the MCH increased from 27.37 to 27.85 (p=0.036). The changes in the other hemogram parameters were statisti- cally in significant. Conclusion: ECT is a safe and an effective and easily appli- cable treatment method with few adverse effects. This study shows that ECT did not produce any significant statistical changes on many hemogram parameters. Two parameters (RBC and MCH) were found that showed significant changes in our study. More studies are needed to clearly understand how ECT changes the red blood cell count and hemoglobin levels.
