KLİNİK ÖRNEKLERDEN İZOLE EDİLEN ENTEROKOKLARDA ANTİBİYOTİK DİRENCİNİN YILLAR İÇİNDEKİ DEĞİŞİMİ

The Change of Antibiotic Resistance Over the Years in Enterococci Isolated from Clinical Specimens

Özet

Enterokokların hem hastane hem de toplum kökenli inf eksiyonlarda önemi giderek artmaktadır. Enterokok türlerinin yol açtığı inf ek- siyonlarda, etkenlerin doğru tanımlanması ve uygun yöntemlerle antibiyotik duyarlılıklarının belirlenmesi kadar antibiyotik direnç prof ille- rindeki değişikliklerin de izlenmesi önem taşımaktadır. Bu çalışmada İzmir Tepecik Eğitim ve Araştırma Hastanesi Tıbbi Mikrobiyoloji Laboratuvarı’nda Ocak 2011-Aralık 2013 tarihleri arasında çeşitli klinik örneklerinden izole edilen Enterococcus f aecalis ve Enterococcus f aecium türlerinin antibiyotik direnç oranlarının belirlenmesi ve bu direncin yıllar içindeki değişiminin araştırılması amaçlanmıştır. Üretilen 3225 enterokokun 2478’i E.f aecalis, 747’si E.f aecium olarak tanımlanmıştır. Ampisilin direnci E.f aecalis’te 2011, 2012 ve 2013 yıllarında sırasıyla % 31, % 14 ve % 5 olarak saptanmış olup yıllar içinde istatistiksel olarak belirgin azalma saptanmıştır (p 0.05). E.f aecalis’te 2011, 2012, 2013 yıllarında sırasıyla vankomisin direnci % 0, % 0.6, % 0.4 ve teikoplanin direnci % 0.5, % 0.6, % 0.7 olarak bulunmuş olup yıllar içinde istatistiksel olarak anlamlı bir değişiklik saptanmamıştır (p>0.05). E.f aecium’da ise 2011, 2012, 2013 yıllarında sırasıyla vankomisin direnci % 17, % 14, % 7 ve teikoplanin direnci % 17, % 13, % 5 olarak bulunmuş olup her iki antibiyotik için 2011-2012 yılları arasında anlamlı bir değişiklik saptanmazken (p>0.05), 2012-2013 yılları arasında istatistiksel olarak anlamlı bir azalma saptanmıştır (p 0.05). Sonuç olarak, hastanemizde E.f aecalis ve E.f aecium türlerinin antibiyotik direncinde yıllar içinde bazı değişiklikler olduğu gözlenmiş olup bu değişikliklerinin tedavide dikkate alınmasının antibiyotik direncinin azaltılmasına katkı sağlayabileceği düşünülmüştür.

Abstract

The importance of enterococci has been increasing in both nosocomial and community acquired inf ections. Not only the accurate identif ication of the agents and the determination of antibiotic susceptibility by appropriate methods but also the monitoring of the changes in antibiotic resistance prof iles has importance in inf ections caused by Enterococcus spp. The aim of this study was to determine the antibiotic resistance rates of Enterococcus f aecalis and Enterococcus f aecium isolated f rom various clinical samples in Medical Microbiology Laboratory of İzmir Tepecik Training and Research Hospital between January 2011-December 2013 and to investigate the change of these resistance rates over the years. Of the 3225 enterococci isolated, 2478 were identif ied as E.f aecalis and 747 were identif ied as E.f aecium. Ampicillin resistance in E.f aecalis was determined as 31 %, 14 %, 5 % respectively in 2011, 2012, 2013 and statistically signif icant decrease was f ound over the years (p<0.05). Ciprof loxacin resistance in E.f aecalis was determined as 19 %, 25 %, 24 % respectively in 2011, 2012, 2013 and statistically signif icant increase was f ound between the years 2011 to 2012 (p<0.05). The resistance rates of E.f aecium f or ciprof loxacin 77 %, 83 %, 81 %, and f or high level gentamicin 69 %, 61 %, 51 % was determined respectively in 2011, 2012, 2013 and there was no statistically signif icant diff erence over the years (p > 0.05). Vancomycin resistance; 0 %, 0.6 %, 0.4 % and teicoplanin resistance; 0.5 %, 0.6 %, 0.7 % was determined respectively in 2011, 2012, 2013 in E.f aecalis strains and there was no statistically signif icant diff erence over the years (p>0.05). Vancomycin resistance; 17 %, 14 %, 7 % and teicoplanin resistance; 17 %, 13 %, 5 % was determined respectively in 2011, 2012, 2013 in E.f aecium strains and there was no statis- tically signif icant diff erence between the years 2011 to 2012 (p>0.05), but statistically signif icant decrease was f ound between the years 2012 to 2013 (p<0.05) in both antibiotics. Linezolid resistance in E.f aecalis; 4 %, 3 %, 2 % and in E.f aecium; 8 %, 5 %, 3 % was determined respectively in 2011, 2012, 2013 and there was no statistically signif icant diff erence over the years (p>0.05). In conclusion, some changes were observed in antibiotic resistance of E.f aecalis and E.f aecium strains in our hospital over the years and it has been thought that the taking into consideration of these changes in the treatment could contribute to the reduction of antibiotic resistance.
