Özet

Uzun süreli dişsizlik, zamanla artan şekilde alveoler kemiğin yüksekliğinde ve genişliğinde azalmaya neden olur ve bu da çenelerde total veya parsiyel atrofiye yol açar. Alveoler sırtlar uygun kemik hacmi açısından yetersiz olduğunda, defekti doldurmak ve yeniden yapılandırmak için ilave cerrahi işlemlere gereksinim durulur. Protetik rehabilitasyon için, defektif alanın non-vaskülarize kemik greftleri kullanılarak yeniden yapılandırılması ve dental implant yerleştirme en yerleşmiş yöntemlerdir. Otojen kemiğin, en etkili kemik grefti olduğuna inanılmaktadır ve günümüzde de “altın standart” olarak kabul görmektedir. Bunun ötesinde, piezocerrahi; osteotomi ve osteoplasti için nispeten daha yeni bir yöntemdir. Mikrovibrazyonlar yumuşak dokuları zedelemeden mineralize dokuların seçici olarak kesilmesini ve çok ince kesilerin yapılabilmelerini sağlamaktadır. Bu olgu bildiriminde, mandibuler knife edge sırtların kortikokanseloz simfizeyal otogreftlerle doldurulması ve piyezoelektrik cerrahi ile kemik genişletme sunulmuştur. Otojen mandibuler kemik greftleri, uygun implant restorasyonu öncesinde mandibulanın posteriorunda kemik sırtlarının genişliğinin artırılmasında güvenilir bir kemik kalitesi sağlayabilir ve final restorasyonun estetik ve fonksiyonel özelliklerini geliştirir. Bununla birlikte, desteklenmemiş ince kemikler problem yaratabilir ve piyezocerrahi ile tek aşamalı kemik genişletme işlemlerinde istenmeyen kemik rezorpsiyonları görülebilir.

Abstract

The long term teeth loss results in progressive loss of alveolar bone height and width so that jaws becomes partially or totally atrophic. When the alveolar ridges lack the appropriate bone volume, additional surgical procedures are needed to reconstruct and augment the deficiency. The reconstruction of defective mandibles using nonvascularized bone grafts and dental implant insertion has become a well-established procedure for prosthodontic rehabilitation. Autogenous bone is believed to be the most effective bone graft material and is still regarded as the “gold standard”. Moreover, piezosurgery is relatively a new technique for osteotomy and osteoplasty. The microvibrations allow a selective cut of only mineralized structures without damaging soft tissues and allow very thin cuts.In the case reports, mandibular knife edge ridges’ augmentation with corticocansellous symphyseal autografts and bone expanding with piezoelectric surgery is presented. Autogenous mandibular bone grafts can provide a reliable bone quality for augmenting the ridge width of the posterior mandible prior to adequate implant restoration and improve the esthetics and function of the final restoration. However, the unsupported thin bones can be problematic and undesirable bone resorptions can occur with one stage surgeries in bone expanding with piezosurgery
