EVALUATION OF CAUSES AND LEVELS OF ANXIETY IN MOTHERS OF INFANTS WITH CLEFT LIP AND PALATE

Özet
Amaç: Bu çalışmanın amacı, dudak damak yarıklı (DDY) bebeğe sahip annelerin ortodonti kliniğine ilk başvurduklarında, nazoalveolar şekillendirme (NAŞ) tedavisi esnasında ve dudak operasyonu öncesinde kaygı düzeylerini ve nedenlerini değerlendirmektir. Bireyler ve Yöntem: Dudak damak yarıklı bebeğe sahip 31 anneye (yaş ortalaması: 27.6 ±5.8 yıl) ölçü alımından önce (T1), NAŞ tedavisinden 4 hafta sonra (T2) ve dudak operasyonundan önce (T3) 10 sorulu anket uygulanmıştır. Annelerin ayrıca Stress Düzey Ölçek Anketi- SDÖ (State-Trait Anxiety Inventory= STAI) anketine cevap vermeleri istenmiştir. Bulgular: Yapılan anketlerde, kaygı düzeyinin ortalama değerlerinde T1 döneminden T2 dönemine geçildiğinde azalma (p 0.05). SDÖ ile T1 ve T3 anket verileri (sırasıyla p= 0.006 r= 0.483, r= 0.445 p= 0.012) arasında pozitif yönde anlamlı bir ilişki belirlenmiştir. Annenin yaşı, eğitim düzeyi, yarık tipi ve ailede dudak damak yarığı hikayesi ile anketlerinin ve SDÖ verileri arasında anlamlı bir ilişki tanımlanmamıştır (p> 0.05). Sonuç: Annelerin kaygı düzeyleri ilk ortodonti randevusunda yüksektir; ancak hekimin bilgi vermesi ve NAŞ tedavi sonuçlarının görülmesi ile azalmaktadır. Bununla birlikte, annelerin kaygısı cerrahi öncesi yeniden artabilir. Kaygı düzeyinin azaltılması amacıyla, tedavi süreci hakkında aile daha fazla bilgilendirilebilir ve tedavi süresi boyunca profesyonel psikolojik destek alabilir.
Abstract
Background and Aim: The aim of this study was to evaluate the causes and levels of anxiety in mothers of infants with cleft lip and/ or palate (CLP) at the initial visit to the orthodontic clinic, during nasoalveolar molding therapy (NAM) and before lip surgery. Subjects and Methods: A 10- item questionnaire to evaluate the causes and levels of anxiety was distributed to 31 mothers (mean age: 27,6±5,8 years) having babies with CLP before taking impression (T1), after 4 weeks from the beginning of NAM (T2) and before lip surgery (T3). The mothers were also asked to answer State-Trait Anxiety Inventory (STAI). Results: According to the questionnaire, mean anxiety level values decreased from T1 to T2 periods (p< 0.01), and increased from T2 to T3 (p< 0.01). No statistically significant differences were found between the mean values of T1 and T3 (p> 0.05). Significant positive correlation was evident between STAI and T1 and T3 questionnaire scores (p= 0,006 r= 0,483, p= 0,012 r= 0,445, respectively). Neither of the questionnaires nor STAI scores were found to be associated with mothers age, educational status, family history or type of CLP. Conclusions: The anxiety levels of mothers were high at their first orthodontic appointment, but decreased as they were informed and observed the results of NAM. However, the anxiety of the mother can increase again before the surgery. In an attempt to decrease the level of anxiety, the family could be further informed regarding the treatment process and get professional psychological support during treatment.
