E-TEST YÖNTEMİYLE METALLO-BETA-LAKTAMAZ VARLIĞI SAPTANAN ACINETOBACTER BAUMANNII SUŞLARINDA AMİNOGLİKOZİD DİRENCİNİN ARAŞTIRILMASI

Özet

Dirençli Acinetobacter inf eksiyonlarında aminoglikozidler diğer antimikrobiyal ajanlarla kombine olarak kullanılabilmektedir. Sınıf B beta-laktamaz olan metallo-beta-laktamaz (MBL) enzimi üreten Gram negatif bakterilerin klinik örneklerden izole edilme sıklığı gittikçe art- maktadır. MBL enzimi monobaktamlar hariç bütün beta-laktamların hidrolize olmalarına neden olabilmektedir. MBL’yi kodlayan genler genelde integronlarla taşınıp, yüksek derecede aktarılabilir özelliğe sahiptir. MBL enzimini kodlayan gen kasetleri ayrıca aminoglikozid grubu antibiyotiklerin direnç gelişimine neden olan gen kasetlerini de taşırlar. Bu çalışmada, kan kültürlerinden izole edilen Acinetobacter bauman- nii suşlarında MBL üretimi ile aminoglikozid direnci arasındaki ilişkinin saptanması amaçlanmıştır. Ocak 2012-Haziran 2013 tarihleri arasında Atatürk Üniversitesi Araştırma Hastanesi’nde kan kültüründen izole edilen toplam 74 A.baumannii suşu çalışma kapsamında incelenmiştir. Birden f azla üremesi olan hastaların sadece ilk üremesi çalışmaya dahil edilmiştir. Tür düzeyinde tanımlama konvansiyonel yöntemlerle ve VITEK 2 compact (bioMerieux, France) otomatize sistemi kullanılarak yapılmıştır. Antibiyotik duyarlılıkları ise Kirby-Bauer disk dif üzyon yöntemiyle belirlenmiştir. Amikasin, tobramisin, gentamisin ve imipenem inhibisyon zon çapları Clinical and Laboratory Standards Institute (CLSI) kriterleri dikkate alınarak değerlendirilmiş, imipenem direnci saptanan suşla- rın MBL üretimi imipenem/imipenem-EDTA içeren E-test stripleri kullanılarak f enotipik olarak tespit edilmiştir. Test edilen 74 A.baumannii suşunun 46’sı (% 62.2) imipeneme dirençli, 28’i (% 37.8) imipeneme duyarlı bulunmuştur. İmipeneme dirençli suşların tamamında MBL varlığı saptanmıştır. En yüksek aminoglikozid direnci MBL pozitif suşlarda % 80.4 oranında gentamisine; MBL negatif suşlarda ise % 25 oranında amikasine ve yine % 25 oranında gentamisine karşı saptanmıştır. MBL pozitif suşlardaki aminogli- kozid direnci MBL negatif suşlara oranla çok daha yüksek oranda bulunmuştur (p<0.05). Hastanemizde izole edilen A.baumannii suşlarında yüksek oranda MBL üretimi tespit edilmiş olup bu suşlardaki gentamisin direncinin- de yüksek olması nedeniyle söz konusu antibiyotiğin kullanımının kısıtlanması uygun bir yaklaşım olacaktır.

Abstract

Aminoglycosides might be used in combination f or treatment of resistant Acinetobacter strains. Metallo-beta-lactamase (MBL), a Class B beta-lactamase, producing Gram negative rods have been increasingly isolated f rom clinical specimens. MBL can hydrolyze beta- lactams f rom all classes except the monobactams. MBL encoding genes are generally transf erred by integrons and have highly transf errable. MBL coding gene cassettes that may also carry the gene cassettes causing aminoglycoside resistance. The aim of this study was to determine the relationship between MBL production and aminoglycoside resistance of Acinetobacter baumannii strains isolated f rom blood cultures. A total of 74 A.baumannii strains isolated f rom blood cultures in Atatürk University Research Hospital were examined in this study during the period f rom January 2012 to June 2013. Only the f irst isolated strains f or each patient were included. Isolates were identif ied by conventional methods and automated VITEK 2 compact system (bioMerieux, France). Antimicrobial susceptibility was determined by Kirby- Bauer disc diff usion method. Antimicrobial susceptibility to amikacin, tobramycin, gentamicin and imipenem were evaluated according to Clinical and Laboratory Standards Institute (CLSI) guidelines. The MBL production of imipenem resistant strains was tested phenotypically using E-test stripes which contained imipenem/imipenem-EDTA. Forty-six (62.2 %) of the 74 A.baumannii strains were imipenem resistant; whereas twenty-eight (37.8 %) of them were sensitive MBL production was f ound in all of the imipenem resistant strains. 80.4 % of MBL producing strains were resistant to gentamicin; whereas in non MBL producing strains resistance ratio were 25 % to amikacin and 25 % to gentamicin. The resistance of MBL producing Acinetobacter strains against the aminoglycoside antibiotics was signif icantly higher than the resistance of the non-MBL producing Acinetobacter strains (p<0.05). It was concluded that the usage of gentamicin should be restricted in our hospital due to high resistance rates f or mentioned agent, especially in MBL producing Acinetobacter strains.
