Nevşehir ilinde hepatit C virüs genotip dağılımı ile serum alanin aminotransferaz ve kantitatif serum HCV rna düzeyleri ilişkisi

Özet

Hepatit C virüsü (HCV) Flaviviridae ailesinin bir üyesidir ve RNA yapısındaki genom yüksek genetik heterojeniteye sahiptir. HCV infeksiyonunun tedavi süresi ve tedaviye verilen yanıt ile genotipleri yakından ilişkilidir. Bu çalışmada Nevşehir ilindeki kronik Hepatit C hastalarında genotip dağılımları ve serum alanin aminotransferaz (ALT), HCV RNA kantitatif düzeyleri ile ilişkisinin belirlenmesi amaçlanmıştır. Çalışmada kronik hepatit C nedeni ile takip edilen 170 hastanın serum örnekleri incelenmiştir. Viral yük Cobas AmpliPrep/Cobas Taqman 48 (Roche, ABD) sistemi, HCV genotipleri ise Real Time HCV Genotype II (Abbott Molecular, ABD) sistemi ile çalışılmıştır. Hastaların 78’i (% 45.1) genotip 1, 64’ü (% 37) genotip 1b, 25’i (% 14.5) genotip 2, ikisi (% 1.2) genotip 3 ve bir hasta (% 0.6) genotip 4 olarak belirlenmiştir. Sonuç olarak, genotip dağılımları ile ALT ve HCV RNA PCR düzeyleriyle arasında istatistiksel anlamlı bir ilişki saptanmamıştır (p>0.05). Nevşehir’de HCV genotip 2 prevalansının ülkemiz genelinden daha yüksek oranda bulunması, bu bölgede daha kapsamlı çalışmalara gereksinim olduğunu göstermektedir.

Abstract

Hepatitis C virus (HCV) is a member of Flaviviridae family and its RNA genome has high genetic heterogenity. In HCV infections, duration of treatment and the response to therapy are strongly related to HCV genotypes. In this study we aimed to identify the distribution of hepatitis C virus genotypes in patients with chronic hepatitis C infection and its associa - tion with serum alanine aminotransferases (ALT) and quantitative serum HCV RNA levels in Nevşehir city. Serum samples of 170 patients with chronic HCV infection were analyzed. Viral quantative RNA was determined by Cobas AmpliPrep/Cobas Taqman 48 (Roche, USA) system and viral genotypes were determined by Real Time HCV Genotype II (Abbott Molecular, USA) system. Genotype 1 was identified in 78 (45.1 %), genotype 1b in 64 (37 %), genotype 2 in 25 (14.5 %), genotype 3 in two (1.2 %) and genotype 4 in one (0.6 %) of the patients. As a result there was no statistically significant relationship bet- ween genotype distribution, HCV RNA load (p>0.005) and ALT levels. The prevalence of HCV genotype 2 was found to be higher in Nevşehir than the average in Turkey. The results of this study indicated that further comprehensive studies are needed in this region.
