2012-2014 YILLARINDA ALT SOLUNUM YOLU ÖRNEKLERİNDEN İZOLE EDİLEN STREPTOCOCCUS PNEUMONIAE VE HAEMOPHILUS INFLUENZAE SUŞLARINDA ANTİBİYOTİKLERE DUYARLILIĞIN YILLARA VE YAŞ GRUPLARINA GÖRE DEĞERLENDİRİLMESİ

Özet
Streptococcus pneumoniae ve Haemophilus influenzae bakteriyel alt solunum yolu infeksiyonu etkenleri arasında önde gelmeleri ve tedavileri için kullanılan antibiyotiklere karşı geliştirdikleri direnç nedeniyle klinik önemlerini korumaktadırlar. Ampirik tedavi seçeneklerinin belirlenmesi için direnç epidemiyolojisinin izlenmesi gerekmektedir. Çalışmamızda 2012-2014 yıllarında alt solunum yolu örneklerinden izole edilmiş S.pneumoniae (n = 223) ve H.influenzae (n = 363) suşunun antimikrobiyal duyarlılıklarının değerlendirilmesi amaçlanmıştır. İncelenen S.pneumoniae suşlarının % 92.4'ü penisiline, % 45.7'si eritromisine, % 56.5'i klindamisine, % 48.4'ü trimetoprim-sülfametoksazole, % 49.8'i tetrasikline duyarlı bulunmuştur. H.influenzae suşlarının % 63.4'ü ampisiline, % 90.1'i ampisilin-sulbaktama, % 89.3'ü sefuroksime, % 61.7'si trimetoprim-sülfametoksazole duyarlı bulunmuştur. Beş yaş ve altı hasta grubunda S.pneumoniae suşlarının eritromisin, klindamisin, trimetoprim-sülfametoksazol ve tetrasiklin duyarlılığının diğer yaş gruplarına göre belirgin farklılık gösterdiği saptanmıştır (sırasıyla p 0.5). Çalışma sonuçlarımız ile solunum yolu infeksiyonu etkeni S.pneumoniae ve H.influenzae suşları için farklı yaş gruplarında ampirik tedavide kullanılan antibiyotiklerin etkinliklerinin değerlendirilmesine olanak sağlayan kümülatif antimikrobiyal duyarlılık verisi oluşturulmuştur
Abstract
Streptococcus pneumoniae and Haemophilus influenzae are still considered to be clinically important pathogens as they are among the leading causes of bacterial infections in lower respiratory tract and have the ability to develop resistance to antimicrobials used for the treatment. In order to determine the empirical treatment options, resistance epidemiology should be monitored. In this study we aimed to evaluate the antimicrobial susceptibility of S.pneumoniae (n = 223) and H.influenzae (n = 363) strains isolated from lower respiratory tract samples between 2012-2014. The susceptibility rates were determined as 92.4 % for penicillin, 45.7 % for erythromycin, 56.5 % for clindamycin, 48.4 % for trimethoprim-sulfamethoxazole and 49.8 % for tetracycline among the S.pneumoniae isolates. The susceptibility rates were determined as 63.4 % for ampicillin, 90.1 % for ampicillin-sulbactam, 89.3 % for cefuroxime and 61.7 % for trimethoprim-sulfamethoxazole among the H.influenzae isolates. The susceptibility rates for erythromycin, clindamycin, trimethoprim-sulfamethoxazole, and tetracycline susceptibility rates in S.pneumoniae isolates were found significantly different in patients ≤ 5 years of age compared to patients in other age groups (p 0.05). Our findings enabled us to generate a cumulative antimicrobial susceptibility test data for S.pneumoniae and H.influenzae strains isolated from respiratory tract infections which may be useful in the evaluation of the efficacy of antimicrobials used in the empirical treatment in different age groups
