Postmortem örneklerden izole edilen staphylococcus aureus suşlarında metisilin, vankomisin, teikoplanin ve makrolid-linkozamid-streptogramin b (mls b ) direnci

Özet
Stafilokokların etken olduğu infeksiyonlarda etkili antibiyoterapi, bakterilerin direnç durumlarının belirlenmesi ile yakından ilişkilidir. Stafilokoklarda metisilin direncinin artışı ve diğer bazı antibiyotiklere direnci de beraberinde getirmesi, bu infeksiyonların tedavisini ve kontrolünü zorlaştırmakta ve ölümlere sebep olabilmektedir. Çalışmamızda Adli Tıp Kurumu Postmortem Mikrobiyoloji Laboratuvarında izole edilen 136 Staphylococcus aureus suşunun metisiline ve glikopeptid antibiyotiklere karşı hassasiyetlerinin araş - tırılması ve makrolid-linkozamid-streptogramin B (MLS B) direncinin belirlenmesi amaçlanmıştır. Clinical and Laboratory Standards Institute (CLSI) önerileri doğrultusunda suşların metisiline direnci Mueller Hinton agar (MHA) besiyerinde sefoksitin (30 μg, Becton- Dickinson) diski ile; MLS B direnci ise, eritromisin (15 μg, Oxoid) ve klindamisin (2 μg,Oxoid) disklerinin merkezleri arasındaki uzaklık 15-26 mm olacak şekilde yerleştirilmesi ile araştırılmıştır. İzolatların vankomisin ve teikoplanine duyarlılıkları E-test yöntemiyle belir - lenmiştir. Tanımlanan S.aureus suşlarının 47si (% 34.6) metisiline dirençli (MRSA), 89u (% 65.4) metisiline duyarlı (MSSA) bulun - muştur. MRSA suşlarının % 17sinde iMLS B direnci, % 2.2sinde M tipi direnç, % 46.8inde ise yapısal tipte direnç saptanırken; MSSA suşlarında iMLS B direnci % 15.7, M tipi direnç % 11.2 olarak bulunmuş yapısal tipte direnç saptanmamıştır. Tüm izolatlar CLSI kri - terlerine göre vankomisin ve teikoplanine duyarlı bulunmuştur. Toplum ve hastane kaynaklı stafilokok infeksiyonlarında çeşitli direnç mekanizmalarının ortaya çıkmasıyla birlikte, tedavi sorun oluşturmakta ve her geçen gün artan düzeyde mortalite karşımıza çıkmaktadır. Bu olguların postmortem değerlendirilmesi, bakteriye ait direnç oranlarının ortaya konulması, epidemiyolojik verilere katkıda bulunacak ve bu infeksiyonların önlenmesi için yapılacak çalışmalara büyük oranda yarar sağlayacaktır.
Abstract
The effective antibiotherapy of infections caused by staphylococci is closely associated with the determination of antibiotic resis - tance. The increase in the methicillin-resistance of staphylococci, which leads to resistance to other antibiotics, renders the therapy and control of staphylococcal infections difficult, which may be fatal. The purpose of this study was to determine the susceptibility to methi- cillin and glycopeptides and resistance to macrolide-lincosamide-streptogramin B (MLS B) of 136 strains of Staphylococcus aureus isola - ted in the Postmortem Microbiology Laboratory of the Forensic Medicine Institute. According to the suggestions of the Clinical and Laboratory Standards Institute (CLSI), methicillin-resistance was assessed on Mueller-Hinton agar (MHA) using cefoxitin disks (30 µg, Oxoid) and MLS B resistance by placing erythromycin (15 µg, Oxoid) and clindamycin (2 µg, Oxoid) disks distanced at 15-26 mm from each other. The susceptibility of the isolates to vancomycin and teicoplanin was determined with the E-test. Of the S. aureus isolates, 47 (34.6 %) were methicillin-resistant (MRSA) and 89 (% 65.4) were methicillin-sensitive (MSSA). Among MRSA isolates, iMLS B resis - tance was determined in 17 %, M-type resistance in % 2.2, and structural type resistance in 46.8 %; among the MSSA strains, the iMLS B and M-type resistance rates were 15.7 % and 11.2 %, respectively, and no structural type resistance was detected. All isolates were sensitive to vancomycin and teicoplanin according to the CLSI criteria. The mortality rates caused by community-acquired and nosocomial staphylococcal infections are increasingly rising due to the emergence of various antibiotic-resistance mechanisms in staph - ylococci. The postmortem evaluation of staphylococcal infections and determination of the antibiotic resistance in those causative staph - ylococci will significantly contribute to epidemiological data, as well as to the prevention of staphylococcal infections.
