FUNGEMİ OLGULARINDA HIZLI BİR ÖN TANIMLAMA TESTİ:POZİTİF KAN KÜLTÜRÜ ŞİŞESİNDEN YAPILAN DOĞRUDANGERM TÜP TESTİNİN DEĞERLENDİRİLMESİ

Özet
Kandidemi, çoğu merkezde sıklığı gittikçe artan ve mortalitesi yüksek bir klinik tablodur. Kandidemilerde en sık etken genellikle Candida albicans olmakla birlikte, diğer Candida türlerinin sıklığında da merkeze göre değişen artışlar bildirilmektedir. Kandidemi tanısı için altın standart kan kültürüdür ve etkenin kesin tanımlaması kan kültürünün pozitif sinyal vermesinden sonra katı besiyerine pasaj yapılarak üreyen mikroorganizmaların konvansiyonel yöntemlerle tanımlanmasını gerektirmektedir. Öte yandan, maya pozitif kan kültürü şişelerinden doğrudan germ tüp testi (GTT) uygulandığında C.albicans ve Candida dubliniensis bir gün önce tanımlanabilmektedir. Bu durumda uygun antifungal tedavinin daha erken başlanması ve mortalitenin azaltılabilmesi için bir fırsat elde edilebilmektedir. Bu çalışmanın amacı, merkezimizde maya pozitif kan kültürü şişelerinden doğrudan GTT değerlendirilmesinin konvansiyonel yöntemlerle uyumunun araştırılmasıdır. Mikoloji Laboratuvarı'na Ekim 2014-Temmuz 2016 arasında gelen maya pozitif kan kültürü şişeleri çalışmaya alınmıştır. Bu şişelerden doğrudan GTT yapılmış, ayrıca Sabouraud dekstroz agar ve kromojenik agara pasaj yapılarak üreyen koloniler konvansiyonel yöntemlerle tanımlanmıştır. Çalışma süresinde 169 hastaya ait 172 kan kültürü örneği değerlendirilmiş, bunlardan 151'inde (% 87.8) doğrudan GTT testi ile konvansiyonel maya tanımlaması sonuçları tam uyumlu bulunmuştur. Doğrudan GTT pozitif olan 80 örneğin tümünde C.albicans/C.dubliniensis üremesi saptanmış, yanlış pozitiflik gözlenmemiştir. Doğrudan GTT negatif olan 92 örneğin ise 11'inde (% 12) C.albicans/C.dubliniensis bulunmuştur. Dikkati çeken bir nokta, örneklerin % 5.8'inde birden fazla maya türünün saptanmış olmasıdır. Bu örneklerden birinde, doğrudan GTT'nin pozitif bulunmasını takiben, C.albicans ile birlikte flukonazole doza bağlı duyarlı veya dirençli olabilen bir tür olan Candida glabrata üremiştir. Sonuç olarak, maya pozitif kan kültürü şişelerinden doğrudan GTT uygulamasının, konvansiyonel tanımlama yöntemleri ile uyumunun yüksek olduğu görülmüştür. Deneyimli personel varlığında kolay uygulanabilen ve maliyeti düşük bir test olan GTT'nin bu şekilde direkt kullanımı, daha erken dönemde uygun antifungal tedavi seçimine olanak tanıyabilmektedir. Ancak, yanlış negatiflikler gözlenebilmesi ve kan kültüründe birden fazla maya türü üremesi durumunda tedavinin yanlış yönlendirilmesi riskinin bulunması nedeniyle, sonucun ön tanımlama sonucu olarak bildirilmesi ve kesin tanımlama sonucunun konvansiyonel testlerden sonra rapor edilmesinin uygun olduğu görüşüne varılmıştır
Abstract
The incidence of candidemia tends to increase in high risk patient populations in most centers and it presents with high mortality. The causative agent is generally Candida albicans; however, frequency of other Candida species is rising with variations among centers. Blood culture is the gold standard to diagnose candidemia and definitive identification of the causative agent requires positive signal in automated blood culture system, subculture on solid media and examination of growth via conventional methods. When germ tube test (GTT) is perfor-med directly from yeast positive blood culture bottles ("direct GTT"), on the other hand, C.albicans and Candida dubliniensis may be identi-fied one day earlier as compared to the conventional methods, providing an opportunity for an earlier appropriate antifungal therapy and decrease in mortality. The aim of this study was to investigate the correlation of the results obtained by direct GTT with those by conventional identification methods. Yeast positive blood culture bottles that were referred to the Mycology Laboratory within the time period of October 2014 and July 2016 were included the study. GTT was performed directly from the bottles and colonies obtained from subcultures on Sabouraud dextrose agar and chromogenic agar were identified by conventional methods. A total of 172 blood culture specimens from 169 patients were evaluated during the study period. Of these, 151 (87.8 %) were in accordance with conventional identification results. C.albicans/C.dubliniensis grew in all 80 direct GTT positive specimens, (leading to no false positives) and 11 of the 92 (12 %) GTT negative specimens. A remarkable point was the detection of multiple yeast species in 5.8 % of the samples. Importantly, one sample for which direct GTT was found to be positive yielded C.albicans together with Candida glabrata, a species known to be less susceptible or putatively resistant to fluconazole. In conclusion, the results obtained by direct GTT from yeast positive blood culture bottles proved to be well correlated with those of conventional identification methods. This particular utility of GTT, which is an easily implemented and low-cost test, might allow earlier guidance for appropriate antifungal therapy. However reporting direct GTT result as a finding of presumptive early identification and confirmation by conventional identification tests remain of particular significance due to the possibility of false negatives and existence of multiple yeast species that may lead to problems in selection of appropriate antifungal therapy.
