AN INVESTIGATION OF DENTISTS’ ATTITUDES TOWARDS CHILDREN WITH DENTAL ANXIETY

Özet

Background and Aim: The aim of the study was to evaluate the attitudes of dentists in Istanbul concerning children with dental anxiety and to define the factors that can affect the dentists’ behaviors. Subjects and Methods: The study was carried out on 463 dental practitioners in Turkey, Istanbul, who answered 24 multiple-choice questions via e-mail or hard copy. The following determinants were tested with the questionnaire: (i) demographic characteristics, (ii) vocational experience of the dentists, (iii) psychological characteristics, (iv) behavior management techniques, and (v) education. All data were analyzed by the SPSS software program (Statistical Package for Social Sciences) version 23.0 using descriptive statistics calculation, Pearson’s chi-square test with p value fixed at 0.05. Results: 34.13% of dentists, reported that they experienced different rates of stress while treating children, 49.24% stated that treating children was difficult, and 21.41% were reluctant to treat children. Dentists with less professional experience felt more stressed when coping with anxious children (p<0.05). Female dentists were more reluctant to treat children with anxiety than were male dentists (p<0.05). While the majority of dentists used basic behavior management techniques (90.6%), 5.41% applied different local analgesia techniques: 2.84% concious/deep sedation, 1.13% general anesthesia, and 0.56% hypnosis. 24.5% of the dentists, who define themselves as “not good” when treating children, are not interested in getting more education in dental anxiety management. Conclusion: During undergraduate education, more detailed and intensive training regarding behavior management techniques may contribute to better quality and easier treatment of children with dental anxiety.

Abstract

Amaç: Bu çalışmanın amacı İstanbul’daki diş hekimlerinin dental anksiyetesi olan çocuklar ile ilgili tutumlarını değerlendirmek ve diş hekiminin davranışlarını etkileyebilecek faktörleri tanımlamaktır. Gereç ve Yöntem: Bu çalışma, e-posta ya da basılı formlar yolu ile 24 çoktan seçmeli soruya cevap vermiş olan 463 diş hekimi üzerinde yürütülmüştür. Diş hekimlerinin tümü İstanbul’da değişik semtlerde özel muayenehane ya da kliniklerde görev yapmaktadır. Anket soruları ile; (i) demografik özellikler, (ii) diş hekimlerinin mesleki tecrübesi, (iii) psikolojik özellikler ve (iv) davranış yönetimi teknikleri gibi belirleyiciler ayrı bölümler halinde sorgulanmıştır. Tüm veriler SPSS programı 23.0 versiyonu kullanılarak analiz edilmiştir. Tüm veriler, SPSS yazılım programı (Sosyal Bilimler için İstatistiksel Paket) 23.0 sürümü ile tanımlayıcı istatistik hesaplaması ve Pearson’ın ki-kare testi kullanılarak analiz edildi, p değeri 0.05 olarak belirlendi. Bulgular: Diş hekimlerinin %34,13’ü çocuk hasta tedavi ederken farklı düzeylerde stres yaşadığını, %49,24’ü çocuk hastaları tedavi etmenin zor olduğunu ve %21,41’i çocuk hastaları tedavi etmede isteksiz olduğunu belirtti. Diş hekimlerinin büyük bir kısmı temel davranış yönlendirme tekniklerini kullanırken (%90,6), %5,41’i farklı lokal anestezi tekniklerini, %2,84’ü bilinçli/derin sedasyonu, %1,13’ü genel anesteziyi ve %0,56’sı hipnozu kullanmaktadır. Kendilerini çocuk hastaları tedavi ederken “iyi değilim” diye tanımlayan diş hekimlerinin %24,5’i dental anksiyete yönetiminde daha fazla eğitim almaya ilgi göstermemektedir. Daha az mesleki tecrübesi bulunan diş hekimleri endişeli çocukları tedavi ederken kendilerini daha stresli hissettikleri görülmüştür (p<0.05). Sonuç: Üniversite eğitimi sırasında, davranış yönetimi teknikleri konusunda daha ayrıntılı ve yoğun bir eğitim, dental anksiyetesi olan çocukların daha iyi kalitede ve daha kolay tedavi edilmesine katkıda bulunabilir.
