Düşük doz lazer tedavisinin sagittal split ramus osteotomisi sonrası meydana gelen duyu değişiklikleri üzerine etkisi

Özet

Amaç: Mandibulayı etkileyen dentofasiyal deformasyonlarda mandibulanın doğru pozisyona getirilmesi için geliştirilmiş􀡤 farklı osteotomiler bulunmaktadır. Bunlar arasında en sık tercih edilen yöntem sagital split ramus osteotomisidir. Bu tedavi sonrası istenmeyen nörosensoriyal bozukluklar oluşmaktadır. Bu bozukluklar; dişlerde, gingivada, alt dudak ve bazen de çene ucunda parestezi, dizestezi, hipostezi şeklinde görülmektedir. Düşük doz lazer tedavisi literatürde biyomodülatör etki üreten bir tedavi yöntemi olarak tanımlanmış olup nörosensoriyal bozukluklarda alternatif tedavi seçeneği olarak popülarite kazanmaktadır. Çalışmamızda düşük doz lazer terapisinin, bilateral sagittal split ramus osteotomisi sonucu oluşan nörosensoriyal değişiklikler üzerine etkisinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışmaya 5’i kadın 6’sı erkek, ortalama yaşları 24 (Dağılım aralığı 17- 30 yıl) olan toplam 11 birey dahil edilmiştir. Bu çalışmaya katılan hastaların rastgele seçilen bir tarafına ameliyattan sonra 24, 48 ve 72. saatlerde ve haftada iki kez olmak üzere 3 hafta süreyle düşük doz lazer tedavisi uygulanmıştır. Aynı hastanın diğer tarafına ise plasebo etkisi yaratmak amacıyla lazer probu cihaz kapalıyken uygulanmıştır. Nörosensoriyal değişiklikler aynı cerrah grubundan ameliyat öncesinde, ameliyattan hemen sonra 15. gün, 30. gün, 2. ay ve 6. ayda pamuk pelet testi ve iki nokta ayırım testi kullanılarak değerlendirilmiştir Bulgular: Plasebo ve uygulama taraflarına ait skor ortalamaları arasında anlamlı farklılık bulunmamıştır. 6. ayda her iki grupta nörosensoriyal değişikliklerin tamamında iyileşme görülmüştür. Sonuçlar: Bu çalışmanın sonuçlarına göre nörosensoriyal değişiklikleri değerlendirmede kullanılan testlerin sonuçlarına düşük doz lazer terapisinin istatistiksel olarak anlamlı etkisi bulunmamaktadır. Bununla beraber psikolojik açıdan iyileşme sürecinde olumlu etkileri göz önünde bulundurulduğunda basit, ekonomik ve non invaziv bir teknik olan düşük doz lazer tedavisinin, bilateral sagittal split osteotomisi sonrası standart bir tedavi protokolü halinde uygulanması önerilebilir.

Abstract

Aim: There are various surgical methods to correct mandibular deformities. One of the most used method is sagittal split ramus osteotomy. Undesirable neurosensorial disorders like paresthesia, dysesthesia, hiposthesia of the teeth, gingiva, chin and lips may occur after this surgical treatment. Low level laser therapy is an alternative, popular and helpful theraphy in treatment of neurosensorial disorders by producing biomodulatory effect. In our study, we evaluated the effect of low level laser theraphy on the neurosensorial changes after orthognatic surgery. Material and Method: 11 patients (5 woman and 6 male ), where the mean age is 24 ( 17-30 years) were included in this study. One randomly selected side of the patients underwent low-level laser treatment at 24, 48 and 72 hours after surgery, and twice weekly for 3 weeks. On the other side of the same patient, the laser probe was applied with the device switched off to create a placebo effect. Neurosensory changes were evaluated by the same surgeon preoperatively, immediately after surgery using cotton pellet test and two-point discrimination test at 15th day, 30th day, 2nd month and 6th month. Results: No significant difference was found between the mean scores of placebo and administration sides. At 6 months, both groups showed improvement in all of the neurosensorial changes. Conclusion: According to the results of the study, no additional treatment is needed to facilitate the healing process of neurosensory changes, but considering the positive effects of the psychological healing process, low-dose laser treatment, which is a simple, economical and non-invasive technique after BSSRO, may be recommended as a standard treatment protocol.
