Akut apandisit tanısında ultrasonografi ve bilgisayarlı tomografi bulgularının karşılaştırılması

Özet
Amaç: Akut apandisit tanısında ultrasonografi (US) ve bilgisayarlı tomografinin (BT) tanıya katkılarının, birbirlerine üstünlüklerinin ve sınırlılıklarının ortaya konması amaçlandı. Gereç ve Yöntem: Klinik ve laboratuar bulguları akut apandisit açısından şüpheli bulunup radyoloji bölümüne gönderilen 35 olgu prospektif olarak çalışma kapsamına alınmış olup tümüne kompresyon US ve helikal BT tetkikleri uygulanmıştır. Akut apandisit tanısında primer kriter olarak US incelemede dilate ve komprese olmayan apendiksin gözlenmesi BT’de ise periapendiküler inşamatuar değişimleri bulunan dilate (6 mm) apendiksin görülmesi kabul edildi. Bulgular: Klinik tanılar olgularımızın 24’ünde cerrahi-patolojik sonuca göre, 11’inde klinik ve laboratuar bulguları eşliğinde 24-48 saat süreyle takip edilerek konuldu. 21 hasta akut apandisit tanısı aldı. Akut apandisit tanısı alan hastaların 20’sinde BT, 16’sında ise US ile tanı konulabilmekteydi. 4 hastada sadece BT ile tanı konulabilmiş, 1 hasta da ise hem US hem de BT ile tanı negatifti. Apandisit tanısı almayan kalan 14 hastanın 5’ine BT ile alternatif tanılar konuldu. Normal apendiks BT’de 7 ve US’de 4 hastada gözlenebildi. US ve BT ile apandisit tanısı alan 1 hastada semptomlar spontan geriledi ve opere edilmedi. BT ve US sonuçları 4 hastada uyumsuz bulundu. Sonuç: Klinik açıdan apandisit şüpheli hastaların tanısının doğrulanmasında, kompresyon US ve helikal BT diagnostik değeri yüksek imaj modaliteleridir. Ancak BT’nin sonuçları US’den daha iyi görülmektedir.
Abstract
Objective: The aim of this study was to compare advantages, accuracy and limitations of ultrasonography (US) and computed tomography (CT) in the definitive diagnosis of acute appendicitis. Materials and Methods: Clinically suspected 35 cases of acute appendicitis according to their physical examinations and laboratory findings were prospectively evaluated by compression US and helical CT. For US examinations a dilated and uncompressible apendix for CT scans a dilated (6mm.) apendix, associated with periapendicular inflamatory reactions were primary criterias. Results: Final diagnoses were made by surgico-pathological results and clinical observations up to 24-48 hours with clinical and laboratory findings. In 21 patients, final diagnosis were acute appendicitis. Correct diagnosis was made by CT in 20, and by US in 16 of these 21 patients. Both CT and US were negative in 1 patient. In 5 of 14 patients which acute appendicitis were not the final diagnosis, CT was able to made alternative diagnoses. Normal apendix could be observed in 7 patients with CT and in 4 patients with US. In 1 patient in whom acute appendicitis was strongly suggested by CT and US, spontaneous regression occurred and surgery was not necessary. CT and US results of 4 patients did not correlate with the clinical diagnosis. Conclusion: Diagnostic value of helical CT and compression US are both high in diagnostic verification of patients in whom clinically acute apendicitis is suggested. But results of CT seem to be more accurate in diagnosis.
