İrritabl barsak sendromu tanısı konulan hastalarda tiroid fonksiyon testlerinin irdelenmesi

Özet

Amaç Bu çalışmada irritabl barsak sendromu tanısı konulan hastalarda tiroid fonksiyon testlerinin durumunun belirlenmesi ve irdelenmesi amaçlanmıştır. Gereç ve Yöntem: Bu amaçla toplam 24 hasta çalışmaya alındı ve iki gruba ayrıldı. Grup 1’de Roma II kriterleri ile tanısı konulmuş 15 (8 kadın) irritabl barsak sendromu tanılı hasta yer aldı. Grup 2’de ise, rastgele seçilmiş, farklı nedenlerle başvuran, sistemik hastalığı ve gastrointestinal semptomları olmayan toplam 9 (7 kadın) hasta çalışmaya alındı. Tüm hastalardan aynı merkezden serbest T3, serbest T4, T3, T4 ve TSH düzeyleri istendi. Bulgular: Grup 1’de yer alan hastaların yaş ortalamaları erkek hastalardan anlamlı olarak daha düşüktü (p0.005). Grup 2 hastalardan sadece birinde hipotiroidizm ile uyumlu hormon profili saptandı. Serbest T3, serbest T4 değerleri açısından her iki grup arasında anlamlı bir fark yoktu (sırasıyla p0.06, p0.06). Grup 1 hastaların 4’ünde TSH düzeyi anlamlı olarak düşük saptandı (0.184±0.01 uIU/ml, p0.06). Grup 1 hastalarda T3 değerinin anlamlı olarak düşük olduğu tespit edildi (p0.06). T4 açısından Grup 1 hastalarda anlamlı olmasa da düşme eğilimi izlenmekteydi (p=0.302). Sonuç: İBS tanısı konulurken ayırıcı tanıda metabolik ve hormonal nedenlerin de göz önüne alınması gerektiği ve bu hastalarda tiroid hormonları nda bazı bozuklukların görüldüğü, tiroid hastalığı tanısı için serbest hormon profilinin önemli olduğu kanılarına varıldı. Bu çalışmanı n daha geniş serilerle yapılması görüşü ile çalışma sonlandırıldı.

Abstract

Objective: In this study thyroid function test results in patients with irritable bowel syndrome are investigated and evaluated. Material and Methods: Twenty-four patients were included into study and distributed into two groups. Group 1 had 15 (8 females) patient with irritable bowel syndrome diagnosed by Rome II criteria. Group 2 had 9 (7 females) patients, chosen randomly and applied to the hospital for other reasons that had no systemic disease and gastrointestinal symptoms. Free T3, free T4, T3, T4, TSH levels of all of the patients were done in the same laboratory. Results: Female patients with IBS were significantly younger than male ones (p0.05). From the hormonal test results only one patient in Group 2 was diagnosed to have hypothyroidism. Comparing the free T3 and free T4 test results, no significant difference occurred between both groups (p0.06) TSH level was found significantly low in 4 patients of Group 1 (0.184± 0.01 uIU/ml, p0.06). T3 results were found to be significantly low in Group 1 (p0.06). Although not regarded as significant, a decrease was observed in T4 results in Group 1 patients. Conclusion: It was concluded that for differential diagnosis of IBS, metabolic and hormonal conditions must be taken into consideration. As various thyroid hormone abnormalities were observed in these patients, free hormone profile holds a great importance in diagnosing thyroidal disease. With the results of the study, it was decided to carry out the study on a larger scale.
