Bir sağlık ocağına başvuran tip 2 diabetes mellitus tanılı hastaların epidemiyolojik özellikleri ve prognozu

Özet
AMAÇ: Çalışma bir sağlık ocağına başvuran Tip 2 Diabetes Mellitus (DM) tanısı olan hastaların epidemiyolojik özelliklerini ve prognozu belirlemek üzere gerçekleştirildi. YÖNTEM: Trabzon Erdoğdu Sağlık Ocağı Çatak Sağlık Birimi’nde, Nisan-Mayıs-Haziran 2008 tarihlerinde yapılan çalışma, tanımlayıcı ve kesitsel bir alan araştırmasıdır. Sağlık Ocağına başvuran tip 2 DM tanısı olan ve kontrol amacıyla gelen (55 kişi) hastaların tamamı araştırmaya dâhil edildi. Araştırma grubundaki bireylerin tümüne çalışmanın amaçları anlatılıp, sözlü onayları alındı. Veri toplama aracı olarak, sosyodemografik özellikler, risk faktörleri, kan şekeri tanı kriterleri, beden kitle indeksi, kronik komplikasyonlar ve ayak muayenesini içeren diyabet takip formu kullanıldı. Diyabet takip formu, Erdoğan ve Nahcivan (1999) tarafından yapılandırılan diyabet risk tanılama formu geliştirilerek oluşturuldu. Elde edilen veriler, sayı ve yüzde dağılımları, ortalama, ki kare testi, korelasyon ve varyans analizi kullanılarak değerlendirildi. BULGULAR: Hastaların %85,5’i kadın olup; yaş ortalamaları 59,4±10,7 yıldır. Ortalama tanı yaşı 53,2±12,5, diyabet yılı 6,6±6,6 yıldır. Hastaların %45,5’i obez, %12,7’si morbit obezdir ve %54,5’inin ailesinde diyabet öyküsü bulunmaktadır. Hastaların %56,3’ünün kan glukozu 140 mg/dl ve %54,5’inin HbA1c düzeyi %7 ve üzerindeydi. Lipid parametreleri değerlendirildiğinde; hastaların %52,7’sinde HDL kolesterol (E<35,K<45) tedavi hedefinden düşük, %76,4’ünde LDL kolesterol 100mg/dl ve %49,1’inde total kolesterol 200 mg/dl üzerindedir. Hastaların diyabet yılı ile HbA1c arasında pozitif ilişki bulundu (r=0.291, p=0.031). Kronik komplikasyon olarak, hastaların %56,9’unda hipertansiyon (HT), %25,9’unda koroner arter hastalığı (KAH), %8.6’sında nöropati ve %1,7’sinde retinopati olduğu bulundu. Ayak muayenesinde 47 hastanın (%85,5) ayağına ilişkin bir ya da birden fazla pozitif bulgu ya da yakınma belirlendi. SONUÇ: Sağlık ocağına başvuran tip 2 DM tanısı ile takip edilen hastaların önemli bir kısmında metabolik kontrol sağlanamamıştır. Bu nedenle, hastaların diyabeti kontrol altına alınıncaya kadar diyabetin akut ve kronik komplikasyonları hakkında bireylerin sosyo-ekonomik ve kültürel yaşantısı da göz önüne alınarak gerekli düzenlemeler yapılmalıdır.
Abstract
AIM: The study was designed to investigate the prognosis and the epidemiologic features of people who was diagnosed having type 2 DM and Applied to the Community Health Center. METHOD: This study which was carried out at Çatak Health Branch of Trabzon Erdoğdu Health Center in April, May, and June in 2008 is a descriptive and cross-sectional field study. All of the patients with Type 2 DM diagnosis who came to the Health Center for routine control (55 people) were involved in the study. The aim of the study was explained to each participant in the study and their oral consent was taken. As the data collection instrument, diabetes surveillance form which includes of socio-demographic features, risk factors, blood sugar diagnosis criteria, body mass index, chronic complications and foot examination was used. Diabetes surveillance form was shaped by modifying the diabetes diagnosis form which was structured by Erdoğan and Nahcivan (1999). The data obtained were evaluated through number and percentage distribution, mean, chi square test, correlation and variant analyses techniques. RESULTS: Age mean of the patients was 59.4±10.7 and 85.5% of them was female. Average diagnosis age was 53.2±12.5, average diabetes year was 6,6±6,6. 45.5% of the patients were obese, 12.7% of them morbid obese and diabetes history in the family was about 54.5%. It was found that 56.3% patients had blood glucose level was 140 mg/dl and 54.5% of the patients’ HbA1c level were higher than 7%. When lipid parameter levels of the patients were examined, it was seen that HDL cholesterol level was (E<35,K<45) for 52.7% of the patients and that was lower than the target treatment level; LDL cholesterol level was 100mg/dl for 76.4%; and total cholesterol level was higher than 200 mg/dl 49.1% of the patients. A statistically significant positive correlation was found between diabetes year of the patients and HbA1c level (r=0.291, p=0.031). 8.6 had % neuropathy, 1.7% of them had retinopathy, 25.9% had coronary heart disease, and 56.9% had hypertension as chronic complication. In foot examination, it was found that 85.5% of them had one of or all of the foot problems or complaint. CONCLUSİON: The metabolic control couldn’t be provided in a significant part of the patients diagnosed having type 2 DM in the area of Health Center. For this reason, necessary arrangement should be done about acute and chronic complications of diabetes also taking into consideration individuals’ socio-economic and cultural lifestyles until bringing patients’ diabetes under control.
