Gelişimsel kalça displazili hastalarda Salter innominat osteotomisi sonuçlarımızın değerlendirilmesi

Özet
Amaç: Gelişimsel kalça displazili (GKD) olgularda uyguladığımız Şalter innominat osteotomisi tedavisinin sonuçlarının değerlendirilmesi. Gereç ve Yöntem: Bakırköy Dr. Sadi Konuk Eğitim ve Araştırma Hastanesi Ortopedi ve Travmatoloji Kliniğinde Ocak 2000- Ocak 2007 tarihleri arasında ilk tedavileri açık redüksiyon ve Şalter innominat osteotomisi (SİO) ile yapılan 50 hastanın toplam 56 kalçası çalışmaya alındı. Hastaların 6'sı erkek 44'ü kız idi. Hastaların ameliyat sırasında ortalama yaşı 22.4 (11 - 60) ay idi. Hastaların 44 tanesi tek taraflı, 6 tanesi iki taraflı idi. Hastalara rutin hazırlıklar sonrası, preoperatif traksiyon uygulanmadan standart SİO operasyonu gerçekleştirildi. Ameliyat sırasındaki değerlendirmeye göre gerekli olanlara addüktor tenotomi ve iliopsoas tenotomisi de eklendi. Hiçbir hastaya femoral kısaltma veya derotasyon osteotomisi uygulanmadı. Hastalara 45 gün süreyle pelvipedal alçı, 45 gün süreyle de kalça abduksiyon cihazı (Dennis- Brown tipi) uygulandı. Hastalar ameliyat sonrası ortalama 21.6 (7-44) ay izleme sonrası değerlendirildi. Hastaların tümü klinik olarak modifiye Mac Kay kriterlerine, radyolojik olarak Severin kriterlerine göre, femur başı avasküler nekrozu açısından Kalamchi ve MacEwen's avasküler nekroz değerlendirme kriterlerine göre değerlendirildi. Bulgular: Hastaların ortalama 21.6 (7-44) ay izleme sonrası klinik değerlendirmede 51 kalça (%91.1) grup I (çok iyi), 4 kalça (%7.1) grup II (iyi), 1 kalça (%1.8) grup III (orta) olarak bulundu. Radyolojik değerlendirmede 44 kalça (%78.5) grup I ( çok iyi), 8 kalça (%14.3) grup II (iyi), 3 kalça (%5.4) grup III (orta), 1 kalça (%1.8) grup IV (kötü) olarak değerlendirildi. Femur başı avasküler nekroz 3 kalçada tespit edildi. Tüm kalçalardaki avasküler nekroz Kalamchi-MacEwen sınıflamasına göre Tip 1 olarak değerlendirildi. Bir hastada redislokasyon tespit edildi. Sonuç: Şalter innominate osteotomisi uyguladığımız olguların sonuçları ve literatürle yapılan karşılaştırma, bu tedavi yönteminin 18ay - 6 yaş arası hastalarda GKD tedavisinde etkin bir yöntem olduğunu ve kliniğimiz sonuçlarının literatür ile uyuştuğunu göstermektedir.
Abstract
Objective: The aim of this study was to evaluate the results of open reduction and Şalter Innominate Osteotomy (SIO) for the treatment of developmental dysplasia of the hip (DDH). Materials and methods: In this study, the results of the open reduction and Şalter Innominate osteotomy in 56 hips (50 patients: 44 girls, 6 boys) who had been operated on Bakırköy Dr. Sadi Konuk Training and Research Hospital, Department of Orthopaedics and Traumatology, between January 2000 - January 2007 was evaluated. At the time of surgery, mean age of the patients was 22.4 months (11-60). Forty-four of the patients were unilateral and 6 of them were bilateral. After routine preparations standard Şalter Innominate Osteotomy was performed without preoperative traction. Adductor and iliopsoas tenotomy were added if it was needed at the intraoperative evaluation. Femoral derotation or shortening were not performed. Postoperatively pelvipedal cast was applied for 45 days and hip abduction brace (Dennis-Brown type) was worn for another 45 days. All of the patients were evaluted after a mean of 21.6 months (7-44) follow-up. The modified Mac Kay criteria were used for the clinical and Severin's criteria was used for radiological evaluation, and the Kalamchi - MacEwen classification was used for the assessment of the developed avascular necrosis of the femoral head. Results: The mean follow up of the patients was 21.6 months (7-44). Of the 56 hips who were operated, 51 of them (91.1%) were group I (very good), 4 of them (7.1%) were group II (good), 1 of them (1.8%) was group III (average) clinically. Of the 56 hips, 44 of them (78.5%)were group I (very good), 8 of them (14.3%) were group II (good), 3 of them (5.4%) were group III (average), 1 of them (1.8%) was group IV (bad) radiologically. Avascular necrosis (3 hips 4.92% and type 1) and redislocation (1 hip 1.8%) were detected postoperatively. Conclusion: The Şalter Innominate Osteotomy was an effective surgical treatment technique for the DDH patients of 18-60 months age and the results of our study were parallel with the successfull results of the literature.
