Üreteroskopik litotripsi sonrası her zaman üreteral stent yerleştirmek gerekir mi?

Özet
Amaç: Kliniğimizde üreter taşı nedeniyle yaptığımız üreteroskopik litotripsi işlemlerinin sonuçlarını incelemek ve ameliyat sırasında üreteral stent yerleştirmenin gerekliliğini tartışmak. Gereç ve Yöntem: Kliniğimizde 2004-2007 yılları arasında, üreter taşı nedeniyle üreteroskopik pnömotik litotripsi uygulanan 161 hasta, taşlarının konumu ve büyüklüğü, müdahale öncesi ve sonrası üreteral ve pelvikaliektazi derecesi, üreteral stent konulma durumu ve sonuçlar açısından incelendi. Bulgular: Olguların 121’i erkek, 40’ı kadındı. Ortalama yaş 37,4±16,2 idi. Üreter taşlarının 84’ü sağda, 71’i solda ve 6’sı iki taraflı idi, toplam 167 üreteroskopi yapıldı. Bunların da 86’sı alt, 67’si orta ve 14’ü üst bölüm taşı idi. Olguların 64’ünde taşın proksimalindeki üriner sistemde dilatasyon mevcuttu. Altmış üç olguda üreter taşının toplam boyutu 10 mm veya üzerinde idi. Olguların 22’sine üreteroskopide darlık görülmesine veya litotripside üreterde travma gelişmesine bağlı olarak üreteral stent yerleştirildi. Yüz kırk beş olguda üreteral stent kullanılmasına gerek görülmedi. Üreteroskopik pnömotik litotripsiden bir gün sonra yapılan direk üriner sistem grafisi ile taşsızlık veya klinik önemsiz taş parçası kalma oranı, 15 gün ve üç ay sonra yapılan ultrasonografi ile de üst üriner sistem dilatasyonu kontrol edildi. Üreteroskopik pnömotik litotripsi uygulanan ve işlem sırasında üreterde darlık saptanmayan, üreter hasarı gelişmeyen, bu yüzden de üreteral stent uygulanmasına gerek görülmeyen olguların yalnızca 3 tanesine işlem sonrası ciddi ağrı ve dilatasyonda gerileme olmaması nedeniyle üreteral stent konulmak zorunda kalındı. Sonuç: Üreteroskopik litotripsi sonrası üreteral stent konulması ciddi derecede üreter darlığı saptanan ve litotripsi sırasında üreter hasarı gelişen olgular dışında zorunlu değildir. Bu yaklaşım, maliyeti de azaltmaktadır.
Abstract
Objective: We aimed to search for the outcome of the ureteroscopic procedures performed for ureteral stones at our department, and discuss the necessity of intraoperative ureteral stent placement. Material and Methods: One hundred and sixtyone cases who underwent ureteroscopic ureterolithotripsy between the years 2004 and 2007 were evaluated in terms of location and size of the stones, pre and postoperative ureteral and calyceal dilatation, use of ureteral stents, and the outcome. Results: Our series included 121 males and 40 females whose mean age was 37,4±16,2 years. Eightyfour and 71 of the stones were on the right and left sides respectively whereas 6 patients had bilateral. Stones were located in the lower, middle, and upper part of the ureter in 84, 67, and 10 patients, respectively. Sixtyfive of the cases had proximal dilatation. Stones were ≥10 mm in 63 patients. Ureteral stents were placed in 22 patients due to the ureteral strictures or traumatic procedure. Ureteral stents were not required in 145 cases who did not have above-mentioned indications. A direct urinary system radiograph for residual fragments on the first postoperative day, and an ultrasound for upper tract dilatation on the 15th postoperative day and thirth month were obtained. Of the patients in whom no ureteral stent was placed intraoperatively, 3 needed stents due to severe pain and/or persistent upper tract dilatation (suspicion for obstruction) postoperatively. Conclusion: Routine intraoperative ureteral stent use during a ureteroscopic lithotripsy is not required except for traumatic procedures and patients with severe ureteral strictures. This is a cost-effective approach as well.
