Fakoemülsifikasyon cerrahisi sonrası erken dönem göz içi basıncı kontrolünde oral ve topikal karbonik anhidraz inhibitörlerinin etkinliği

Efficacy of oral and topical carbonic anhydrase inhibitors in controlling early period postoperative intraocular pressure elevation after phacoemulsification surgery

Özet

Amaç: Fakoemülsifikasyon cerrahisi sonrası erken dönemde göz içi basıncı (GİB) üzerine, oral asetazolamid 250 mg, topikal %1 brinzolamid ve %2 dorzolamid uygulamalarının etkilerinin karşılaştırılması. Gereç ve Yöntem: Çalışma, kliniğimizde aynı teknik ve operasyon şartları altında, komplikasyonsuz fakoemülsifikasyon cerrahisi uygulanan, başka herhangi bir patolojisi olmayan 68 hastanın 80 gözünde prospektif olarak planlandı. Hastaların 38’i erkek 30’u kadındı. Hastalar sağ veya sol gözleri, yaşları ve cinsiyetleri göz önüne alınmaksızın randomize olarak 4 gruba ayrıldı. Operasyonun hemen bitiminde, postoperatif 12. ve 24. saatlerde, 1. gruba topikal dorzolamid %2, 2. gruba topikal brinzolamid %1, 3. gruba oral 250 mg asetazolamid uygulandı. Kontrol grubu olan 4. gruba hiçbir tedavi uygulanmadı. Hastaların GİB’ları, operasyon öncesi, operasyon sonrası 4. ve 24. saatlerde Goldmann applanasyon tonometresi ile ölçüldü. Bulgular: Cerrahi öncesi, cerrahi sonrası 4. ve 24. saatlerdeki GİB ölçüm değerleri karşılaştırdığında, Grup 1 ve 2 istatistiksel olarak anlamsız bulunurken (p>0.05), Grup 3 ve 4’de tüm ölçümlerde istatistiksel olarak anlamlıydı ( p 0.05), cerrahi sonrası 4. saat GİB ortalamaları arasındaki fark istatistiksel olarak anlamlıydı (p 0.05). Sonuç: Postoperatif 4. saatte topikal brinzolamid ve dorzolamid, oral asetazolamide göre GİB kontrolü açısından daha etkin iken, 24. saatte benzer etkili bulundu. Topikal karbonik anhidraz inhibitörleri, özellikle sistemik rahatsızlıkları olan ve oral asetazolamid tedavisinin riskli olduğu hastalarda tercih edilebilir.

Abstract

Objective: To compare the effectiveness of oral acetazolamide 250 mg, topical brinzolamide 1% and dorzolamide 2% on intraocular pressure (IOP) during the early period after phacoemulsification surgery. Material and Methods: The study was planned prospectively in our clinic under the same technique and operation conditions, applying uncomplicated phacoemulsification surgery in 80 eyes of 68 patients without another pathology. When 38 patients were men, 30 patients were women. Patients were randomly divided in four groups without thinking their right or left eyes, age, and sex. It was applied dorzolamide 2% for first group, brinzolamide 1% for second group, oral asetazolamide 250 mg for third group after operation immediately, then in 12th hour and 24th hour. It was not applied treatment for fourth group that was controlling group. IOP of patients were measured using Goldmann applanation tonometer preoperatively and 4th hour and 24th hour postoperatively. Results: When before and after surgery 4th hour and 24th hour IOP measurement values compared, group 1 and 2 was found no significantly different (p>0.05), but group 3 and 4 were significant statistically ( p<0.001 ve p<0.05). IOP values that were measured before and after surgery in 4th and 24th. hour were statistically compared. When Group 1 and 3, Group 2 and 3 compared before and after surgery in 24th hour IOP average were not significant statistically (p>0.05), but they were significant statistically postoperative 4th hour (p<0.05). When Group 1 and 2, Group 1 and 4, Group 2 and 4, Group 3 and 4 compared before and after surgery in 4th and 24th hour, it was not observed significant statistically as IOP average (p>0.05). Conclusion: When topical brinzolamide and dorzolamide was more effective than acetazolamide IOP control 4 hours after phacoemulsification, it was found similar effects at 24 hours. Topical carbonic anhydrase inhibitors were preferred especially for patients that had sistemical illnesses and risky oral asetazolamide treatment.
