Kompleks Omurga Cerrahisinde Hücre Koruyucu Sistemin Etkinlik ve Maaliyet Analizi

Özet
Amaç: Kompleks omurga cerrahisi uygulanan olgularda operasyon sırasında kullanılan hücre koruyucu sisteminin, kullanılmayan olgulara göre etkinliği ve maliyeti araştırıldı. Gereç ve Yöntem: Kliniğimizde Ocak 2009-Mart 2013 tarihleri arasında kompleks omurga hastalığı tedavisi amacıyla posterior enstrümantasyon ve füzyon uygulanan hastalar geriye dönük olarak tarandı. Grup Ide operasyon sırasında hücre koruyucu sistem kullanılan 44 hasta, Grup IIde ise kullanılmayan 20 hasta çalışmaya dâhil edildi. Hücre koruyucu sistemi kullanımının operasyon sırasında ve sonrasında allojenik kan transfüzyon ihtiyacı açısından etkinliğinin ve maliyetinin belirlenmesi hedeflendi. Çalışmamızda istatistiksel analizler için IBM SPSS Statistic Version 20.0 programı kullanıldı. Bulgular: Ameliyat sırasında ve sonrasında Grup Ide kullanılan eritrosit süspansiyonu (ES) miktarı ortalama 1±1, Grup IIde ise 2.6±1.8 olarak saptandı ve fark anlamlı bulundu (p=0.004). Kullanılan HKS ve ES maliyetlerine bakıldığında ise grup I de 752.7±148.7 TL, grup II de ise 241.8±169.1 TL olarak saptandı ve fark ileri derecede anlamlı bulundu (p<0.001). Sonuç: Kompleks omurga cerrahisinde hücre koruyucu sistem kullanılan olgularda, kullanılmayanlara göre ameliyat sonrası 1. gün hemoglobin, hematokrit, kırmızı kan hücre sayımı değerleri daha yüksek düzeyde saptanmakta, ve bu sistemin kullanımı ameliyat sırasında ve sonrasında allojenik eritrosit süspansiyonu ihtiyacını anlamlı derecede azaltmaktadır.
Abstract
objective: The efficiency and cost of cell saver system is compared with in patients who underwent to complex spine surgery with and without the system. Material and methods: We restrospectively evaluated the pateints who underwent to posterior instrumentation and fusion in our clinic between Jan 2009-Mar 2013 for treatment of complex spine disease. There were 44 patients in group I which cell saver system was used during operation, and 20 patient in gorup II which system was not used. We aimed to determine the efficiency and cost of the usage of cell saver system against to allogenic blood transfusion in peroperative and postoperative periods. IBM SPSS Statistic Version 20.0 program was used for statistical analyses. Results: The number of erytrosyte suspension used in the peroperative and the postoperative periods was 1±1 in group I, and 2.6±1.8 in group 2 and the differnce was statistically significiant (p=0.004). When compared by the cost, group I was 752.7±148.7 TL and group II was 241.8±169.1 TL with statistically significiant difference (p<0.001). Conclusion: In complex spine surgery, hemoglobin, hematocrit and red blood cell values are higher at the first postoperative day in patients whom cell saver system is used during operation against whom did not used and the usage of this system decreases allogenic blood transfusion at the peroperative and the postoperative periods.
