Özet

AMAÇ: Enterobacteriaceae enfeksiyonları ile mücadelede karşımıza çıkan önemli bir problem, genişlemiş spektrumlu beta laktamaz (GSBL) üretimi ile ilgili direnç oluşumudur. Bu çalışmada mikrobiyoloji laboratuvarımızda izole edilen E.coli suşlarının GSBL sıklığı ve antibiyotik direnç oranlarının belirlenmesi amaçlanmıştır. YÖNTEM: Ocak 2010 - Aralık 2011 tarihleri arasında çeşitli klinik örneklerden izole edilen E.coli suşları retrospektif olarak değerlendirilmiştir. GSBL üreten ve üretmeyen E.coli suşlarında antibiyotik dirençleri ve yıllara göre GSBL oranları karşılaştırılmış ve sonuçlar istatistiksel olarak değerlendirilmiştir. BULGULAR: 2010 yılında %21,9, 2011 yılında ise % 25 E.coli suşunun GSBL ürettiği saptanmıştır. Hiçbir suşta imipenem, meropenem, ertapenem ve tigesikline karşı direnç görülmemiş olup, amikasin hariç diğer antibiyotiklere direnç GSBL üreten suşlarda üretmeyenlere göre anlamlı bulunmuştur (p<0.05). SONUÇ: Surveyans çalışmaları uygun antibiyotik tedavisi ve direnç gelişiminin önlenmesi için yol göstericidir. Lokal surveyans verilerine göre ampirik tedavi başlanmalı ve kültür sonuçlarına göre de-eskalasyon yapılmalıdır.

Abstract

AIM: The resistance related with production of extended-spectrum beta-lactamase (ESBL) is an important problem that we face when we are treating infections caused by enterobacteriaceae. We aimed to evaluate the frequency of ESBL production in E. coli strains and the rate of antibiotic resistance isolated from in our microbiology laboratory. METHOD: E. coli strains isolated from various clinical specimens between January 2010 - December 2011 were assessed retrospectively. Resistances to ESBL-producing and non-producing E. coli and the rate of ESBL production according to the years were investigated. The results were compared with statistical analysis. RESULTS: The rate of ESBL producing E. coli was 21.9% in 2010, and 25% in 2011. There was no resistance to imipenem, meropenem, ertapenem and tigecycline in all strains. The resistance to antibiotics other than amikacin was statistically significant in ESBL producing E. coli compared to ESBL non- producing E.coli (p<0.05). CONCLUSION: Surveillance studies guide appropriate antibiotic therapy and prevent development of resistance. Empiric treatment should be started according to local surveillance data, and de-escalation of the administered antibiotics should be used following culture results.
