Hemiartroplasti ve internal tespit yöntemi ile tedavi edilen kalça kırıklı hastaların değerlendirilmesi

Özet

Amaç: Femur üst uç kırığı nedeniyle kliniğimizde Mayıs 2002-Aralık 2006 arasında internal tespit ve hemiartroplasti ile tedavi edilen 137 hasta fonksiyonel sonuçları, mortalite, komplikasyon ve reoperasyon oranları yönünden değerlendirilmesi ve karşılaştırılması. Gereç ve Yöntem: 137 hasta çalışmaya alındı, 92 hastanın sağ olduğu saptandı. Grup 1 kalça kırığı nedeniyle internal tespit uygulanan 44 hastanın (24 K, 20 E;ort. yaş 66,6±17,2; 27-95) ortalama takip süresi 23±10,8 ay (6-51) idi. Grup 2 kalça kırığı nedeniyle hemiartroplasti ile tedavi edilen 91 hastanın (60 K, 31 E; ort. yaş 73,8±11,2; 49-96) ortalama takip süresi 22,7±15,4 ay (6-54) idi. Bulgular: Harris kalça skoruna göre internal tespit yapılan grupta 18 hastada (%40,3) mükemmel ve çok iyi, 19 hastada (%43,9) iyi ve orta, 7 hastada ise kötü sonuç tespit edildi. Hemiartroplasti yapılan grupta ise 28 hastada (%45) mükemmel ve çok iyi, 27 hastada (%43,5) iyi ve orta, 7 hastada (%11,5) ise sonuç kötü olarak bulundu. Sonuç: İnternal tespit yöntemi ile tedavi edilen kalça kırıklı hastalarda komplikasyon ve tekrar ameliyat edilme oranları yüksek çıksa da mortalite oranları hemiartroplastiye göre düşüktür. Ayrıca internal tespit ve hemiartroplasti ile tedavi edilen hastaların fonksiyonel sonuçları da benzerdir. Bu nedenle özellikle femur intertrokanterik kırıklı hastalarda kalça eklemini koruyucu tedavinin seçilmesi daha uygun olacaktır.

Abstract

Objective: Assessment and comparison of 137 patients who were treated with internal fixation and hemiarthroplasty in our clinic between May 2002 and December 2006 because of proximal femur fracture in terms of functional outcomes, mortality, complication and reoperation ratios. Material and Methods: A total of 137 patients were included in the study, 92 patients were detected to be alive. Group 1 number of the patients who underwent internal fixation because of hip fracture was 44 (24 females, 20 males, mean age 66,6±17,2 years; range 27-95). Mean follow-up time was 23±10,8 months (6-51). Group 2 number of the patients treated with hemiarthroplasty because of hip fracture was 91 (60 females, 31 males, mean age 73,8±11,2 years; range 49-96). Mean follow-up time was 22,7±15,4 months (6-54). Results: According to Harris hip score, excellent and very good outcomes were obtained in 18 (40,3%) patients who underwent internal fixation group, good and moderate outcomes were obtained in 19 (43,9%) patients and poor outcomes were obtained in 7 patients. Of the patients who underwent hemiarthroplasty group, outcomes were found as excellent and very good in 28 (45%) patients, good and moderate in 27 (43,5%) patients and poor in 7 (11,5%) patients. Conclusion: Although complication and reoperation ratios were found high in patients with hip fracture treated with internal fixation method, ratio of mortality was lower compared to hemiarthroplasty. Also functional outcomes of the patients treated with internal fixation and hemiarthroplasty are similar. Thus, preferring the treatment conserving hip joint would be more appropriate especially for the patients with femur intertrochanteric fracture.
